What is the overall goal of hospice care?
to keep pt comfortable as death approaches.
Palliative Care
the active, complete care of a patient whose disease has not responded to curative therapy.
Curative treatment
aggressive care in which the goal is curing the disease and prolong life at all costs.
When is hospice initiated?
when curative measures are no longer effective
What is the criteria for admission to hospice?
- Pts illness is terminal
- life expectancy 6 months or less
- pt desires to be on hospice
- staff may no preform life saving measures
- pt and caregiver must be willing to participate in planning.
Medical director
to function as a mediator between the team and healthcare provider
Is it necessary that a primary caregiver be a relative?
NO.
Does the primary caregiver have complete control over the pt care?
NO. pt may have a power of attorney
Goals of Hospice
- control or relieve pts symptoms
- be involved in decisions of care
- encouraging the pt to live life to the fullest
- providing continuous support
- educating and supporting primary caregiver
Nurse coordinator
Manages the pts care and assesses and admits pt to hospice care
Social worker
Evaluates pts mental wellbeing and provide resources.
Spiritual coordinator
between pt and spiritual community, gives spiritual support
Volunteer coordinator
assigns volunteers to give the family respite
