A

Infants 1-3 months: 0.5mg/kg/dose once daily for up to 8 weeks
* Infants >3months-1year: 1mg/kg/dose twice daily for up to 8 weeks (max dose 40mg/day)
* Children & Adolescents: 1mg/kg/dose every 12h (max dose 40mg/day)
