what are the layers of the pericardium?
fibrous outer layer
serous inner layer (parietal and visceral)
what is the function of the pericardium?
produce lubricant
prevent over dilation
coordinate left/right ventricle contraction
protect heart
helps maintain position in thorax
what are the causes of pericardial effusions in dogs?
idiopathic
neoplastic
what are the causes of pericardial effusion in cats?
congestive heart failure
FIP
infection/neoplasia
what breeds of dog are predisposed to idiopathic pericardial effusions?
(large breeds)
st. bernard
golden retriever
labradors
what is one of the main neoplastic causes of pericardial effusion in dogs?
haemangiosarcoma
how do idiopathic pericardial effusions present?
chronic effusions that slowly build up over time
what is the difference between he presentation of neoplastic and idiopathic pericardial effusions in dogs?
neoplastic - acute (due to haemorrhage)
idiopathic - chronic
where are haemangiosarcomas most commonly found on the heart?
right atrium (check liver and spleen)
what dog breeds are predisposed to haemangiosarcomas?
German shepherd
golden retrievers
setters
how are pericardial effusions from haemangiosarcomas treated?
palliative (drain, removal…)
chemotherapy
what is the prognosis for cardiac haemangiosarcomas?
very poor/grave
what neoplasias can cause pericardial effusions in dogs?
haemangiosarcoma
chemodectoma
mesothelioma
lymphoma
how severe are chemodectomas in dogs?
good prognosis - slow growing, minimally invasive, low metastatic rate, benign tumour
what breeds are chemodectomas most commonly seen in?
brachycephalic breeds
where are chemodectomas found?
on the heart base
what is the prognosis for mesotheliomas that cause pericardial effusions?
poor
which chamber of the heart is most effected by pericardial effusion?
right atrium (the chambers with the lowest pressure will be effected the most by the pressure exerted by the effusion)
what is the pressure exerted on the heart by a pericardial effusion called?
cardiac tamponade
what is pulsus paradoxus?
with pericardial effusions the pulse gets weaker as you breath in you increase right sided filling which squashes the left side so there is less output from the left ventricle and hence a weaker pulse
what sided heart failure occurs with cardiac tamponade?
right sided (right atrium has the lowest pressure so you get increased preload)
what are the clinical signs of pericardial effusions in dogs?
non-specific (decreased appetite, lethargy, GI upsets)
specific (decreased exercise tolerance, cough, syncope, abdominal enlargement from ascites)
how do cats with pericardial effusions present?
congestive heart failure
how does chronic pericardial effusion present in dogs?
right sided congestive heart failure - organomegaly, abdominal effusion, positive hepatojugular reflux, jugular distension, tachycardia
weak femoral pulse
muffled heart sounds
