Mention the role of antidotes in OPC poisoning
How are carbamates different?
1-Atropine antagonizes M but not N effects, may protect from cardiac complications.proper oxygenation ensured before administration (1-2 mg IV every 20 min for adult)
2-Oximes: they reactivate cholinesterase enzymes hence correct M,N and central effects. Protect from intermediate syndrome, atropie pre-ttt is essential
=Carbamates have short duration and do not require oximes.
When is atropine stopped in TTT of OPC toxicity?
- Dryness of pulmonary secrtion
2.relief of bronchospasm and wheezes
BUT NOT HR or pupil size
When to discharge a case of OPC toxicity?
1-2 days after pateint is astmptomatic without requiring administartion of atropine or oximes
Mention phases of phospides toxicity
1st phase: GI, CNS, CVS, respiratory and metabolic symptoms (1-24 hr)
2nd phase: apparent recovery (24-48 hr)
3rd phase: severe toxic hepatitis (36-7 days)
Mention the role of elimination in phosphides toxicity
