1- Scene safety
Ensure the scene is safe for you, other emergency service personnel, your patient’s and the public.
If a scene is unsafe, ensure correct guidance from the guards is followed or superior medical personnel.
1.1 - PPE
-Correct PPE should be worn in all situations and up to date procedures should be followed.
-Place face masks on patient’s when necessary
-Hand washing and hand hygiene should be carried out before and after every patient when able.
-PPE includes, gloves, aprons, goggles, face masks, barriers, foot covers, face shields…
2 - Clinical decision making
A competent patient, when in the right state has the right to refuse treatment and admission to hospital unless otherwise would place the patient in harms way. (Assisted Decision-making (Capacity) Act 2015)
3 - Consent
Seek consent prior to initiating intentions and/or administering medications
4 - Identify and manage
Identify and manage life-threatening conditions
5 - Ventilation
Ensure adequate ventilation and oxygenation
6 - Tissue perfusion
Optimise tissue perfusion (blood flow)
7 - Diagnosis
Make a working diagnosis, after considering differential diagnoses
8 - Pain relief
Provide appropriate pain relief within scope of practice
8.1 - Do not delay
Pain management should not delay the diagnosis of conditions or injuries
8.2 - implement for all relevant patients
Pain management should be implemented to all relevant patients
8.3 - ten minutes
Pain management should commence within ten minutes on scene
8.4 - Tolerable
Pain management should reduce pain to a tolerable level
8.5 - Cognisance (awareness)
Pain management should take cognisance of immediate and short term pain management requirements by administering appropriate combinations of analgesia (The inability to feel pain)
9 - other conditions
Identify and manage other conditions
10 - Posture
Place the patient in the appropriate posture according to their presenting conditions
11 - Body temperature
Ensure maintenance of normal body temperature (Unless a CPG indicates otherwise)
12 - Reassurance
Provide reassurance at all times
13 - Vitals
Monitor and record patient’s vital observations
14 - Responsibility
Maintain responsibility for patient care until handover to an appropriate practitioner
15 - Transport
Arrange transport to an appropriate medical facility, if clinically required, and in an appropriate timeframe
16 - Patient care record
Complete a patient care record following an interaction with a patient
17 - Identify the clinical lead on scene
This will be the most qualified practitioner on scene. In the absence of a more qualified practitioner, the practitioner providing care during transport shall be designated clinical lead as soon as practical.
18 - Decontamination
Ambulances, medical rooms and equipment should be decontaminated as appropriate following an interaction with a patient
