Q

In a post-partum haemorrhage, what is the risk to the pituitary?

A

Sheeman’s syndrome; Pituitary infarct.

This will present as secondary hypothyroidism (Low TSH and Low thyroxine)

MOA: Pregnancy causes increased action of the pituitary (increased production of thyroxine, LH, FSH, prolactin and oestrogen), just like the thyroid gland this means it’s demand for blood increases, so this means that it is very sensitive to poor blood supply.
