Plama Cell Disorders Flashcards
(15 cards)
Plasma cell disorders are monoclonal neoplasm from the
B-lymphocyte lineage
Examples of plasma cell disorders
Multiple myeloma
Waldenstrom macroglobulinemia
Primary amyloidosis
Clinical manifestations of all the plasma cell disorder relate to
- Expansion of the neoplastic cells
- Secretion of cell products (immunoglobulin molecules or subunits, lymphokines)
- Host response to the tumor
In most plasma cells, light chains are cleared by the kidney
Light chains are excreted per day
<10 mg
Immunoelectrophoresis
Qualitative assessment of M component
Electrophoresis
Quantitative assessment of the M Component
Monoclonal antibody must be present at
Concentration of Atleast 5 g per liter to be accurately quantitated by electrophoretic analysis
Amount of M component in the serum
Reliable measure of tumor burden
Excellent tumor marker to manage therapy
Not specific enough to be used to screen asymptomatic patients
Rare skin diseases associated with with a monoclonal gammopathy
Lichen myxedematosus aka papular mucinosis
Necrobiotic xanthogranuloma
Papular mucinosis aka lichen myxedematosus
Highly cationic IgG is deposited in the dermis of patients
Necrobiotic xanthogranuloma
Is a histiocytic infiltration of the skin usually face
Produces red or yellow nodules that can enlarge to plaques
Light chains are secreted in the urine as
Bence Jones Protein
IgG myelomas are more common than IgA and IgD myelomas
