14
Q

Diagnosis of Silicosis

A

ACUTE :
Hilar LNAP
BAL- milky fluid , PAS + flocculent densities
HRCT - Crazy pavement appearance
Ground glass opacities with interstitial septal thickening along with interspersed normal areas

CHRONIC :
CXR- Egg shell calcification in upper lung (<1cm nodules )
- conglomerate masses

15
Q

Cause of productive cough in acute silico proteinosis

A

Secondary alveolar proteinosis
Due to collection of Surfactants as a result decreased macrophage action ( because of exposure to silica dust )

16
Q

Silica reaction

A

Formation of calcified fibrotic nodule in lung parenchyma which causes inflammatory reaction leading to PMF in later stages

17
Q

Silicosis increases risk of

A

TB
COPD
LUNG CANCER
COR PULMONALE
CAPLANS SYNDROME

18
Q

Erasmus syndrome

A

Systemic sclerosis after exposure to SILICA

19
Q

Comet tail sign

A

HRCT finding
Seen in Rounded atelectasis / folded lung syndrome / Blesovsky syndrome

Diffuse pleural thickening pulls bronchoalveolar bundles thus producing sign

Rare complication of Asbestosis

Intense pleural reaction causing folding of lung and pleura

20
Q

Virus ass with mesothelioma

A

Simian virus 40

21
Q

IOC for Asbestosis

A

Thoracoscopy guided biopsy

22
Q

Most specific cancer in Asbestosis

23
Q

Most common lung cancer in Asbestosis

A

Non small cell lung cancer ( adenocarcinoma)

24
Q

Ferrugineous bodies or asbestos bodies , other name

A

Asbestos fibres covered with FERRITIN
Golden beaded appearance on HPE

25
Plaques in Asbestosis ( benign )
Discrete : Clover leaf appearance Masses: candle dripping appearance
26
Extra pulmonary cancers due to Asbestosis
Stomach cancer Laryngeal ca Colon ca
27
Types of asbestos fibres (6)
Chrysolite Crocidolite Amosite Anthophyllite Tremolite Actinolite
28
Mgt of mesothelioma
Pemetrexate + cisplatin ⬇️ Nor response ⬇️ Gemcitabine ( anti metabolite )
