What is endometritis?

A

Poly microbial infection of the endometrium with bacteria from the lower genital tract.

How is pernieal laceration different from episiotomy?

A

perineal laceration is a tear of the skin of the perineum with vaginal child birth.

Episiotomy: intentional incision of the perineum to aid with facilitation of delivery.

A

lacerations extend to the skin and subcutaneous tissue of the perineum, and the perineal muscles are intact

A

Lochia rubra: mense like bleeding with blood & necrotic decidual tissue

Lochia serosa: lighter discharge with less blood (mucopurulent and may have a mild odor)

Lochia Alba: yellow-whitish discharge (may persist for 7wks)

A

Within 2 weeks of delivery the uterus should be approximately at the level of the pelvis. By 6 weeks post partum, it should be normal size.

A

Lactating women = increase prolactin levels = ovulation suppression.

nonlactating: 70-75 days (2 months)
Lactating: 6 months

Why does postpartum ovulation suppression occur?

A

Ovulation suppression is related to elevated prolactin and decreased estrogen levels.

Lactating women: increased prolactin lasts 6 weeks & estrogen levels remain low.

Nonlactating women: prolactin normalizes by postpartum week 3 and estrogen rises

What is the definition of a postpartum fever?

A

A temperature of 100.4° F x2 separate occasions, more than 24 hours after delivery.

A

A complication of endometritis of when a thrombus in the pelvic vein gets infected.

Postpartum endometritis not improved with abx

Will need heparin added to gentamicin & clindamycin

A

>1000ml c-section

A

Tone: Atony “soft & boggy” uterus (MCC)
Trauma: laceration to cervix & vagina
Thrombin: underlying coagulation disorder
Tissue: retained placenta
