Power Point 8 (CP and Myelodysplasia) Flashcards
(18 cards)
What is Myelodysplasia?
Defective development of any part of the spinal cord
What is Myelomeningocele?
Opening in spine and sac
What else is Myelomengocel associated with (pathologies)?
Encephaloceles
Hydrocephalus
Arnold- Chiari II
Another name for Myelomengocel?
Spina bifida
Aperta
occulta
What are the musculoskel deformities associated with Myelodysplasia?
Contractures in LE
Postural Problems
What postural deviations and contractors are seen in high level lesions? (T12-L2)
Hip Flx, Abd, ER contractures
Knee Flx contractures
Ankle PF contracture
Lumbar lordosis
Postural deviations for mid to low lumbar lesions? (L3-L5)
Hip and knee Flx contractures Lumbar Lordosis Genu Valgum Calcaneal Valgus and pronated feet Crouched gait
Postural deviations and contracutres at low level (sacral)?
Mild Hip and Knee Flx contractures
Lumbar Lordosis
Calcaneal Valgus or Varus
Mild crouched gait
Grab bag of other postural deviations and contractors regardless of level
Scoliosis
Kyphosis
Lordosis
Restricted respiratory capacity
What are the goals for Myelodysplasia?
Positioning- No habitual sitting and sleeping postures
Orthotics- Maintain alignment, max muscle effort, prevent deformity and increase function
Surgery- Spinal surgery after puberty. Hip surgery
What is Hyrdocephalus?
Excess CSF in ventricles of brain causing enlarged head.
What is the treatment for Hydrocephalus?
VP (Ventriculo-peritoneal shunt.
What is Arnold- Chiari Malformation?
Cerebellar hypoplasia with caudal displacement of hindbrain through foramen magnum. Sometimes causing non communicating hydrocephalus as a result of obstruction of CSF. Associated with hydrocephalus.
What are signs of shunt dysfunction?
Changes in speech headache decreased activity level irritability decreased grip strength upward eyes doll
Dandy-Walker Syndrome
Congenital brain malformation involving cerebellum and fluid around it. Key feature is partial or complete absence of cerebral vermis.
Unshunted hydrocephalus results in _____?
Enlarged Head but normal face
Difficulty with head control
Thinned brain material
Frontal Bossing
What are sensory deficits?
Decreased Kinesthesia
Don’t always correlate with motor levels
Check Vibratory and either light touch or pinprick
Skin inspection and pressure relief technqiues
What is MOMS and what does it say?
Management of Myelomeningocele Study
Prental surgery benefits outweigh risks. Babies who underwent fetal repair of spina bifida were half as likely not to have shunt and chair malformation
