A

ID SLAP lesion, shoulder
- goal to activate biceps

initial: patient holds arm at 90deg flexion and pronated

try to flex, supinate, and externally rotate all at once
- therapist: holds wrist to resist motion, palpate long head of biceps tendon w other hand

pain and if tendon pops off (transverse humeral ligament tear)

patient: holds arm at 45 deg b/w flexion and extension

ask to turn hand like emptying can onto floor

therapist: one hand on scapula, other on upper arm to resist motion

BILATERAL, looking for weakness

A

searching for AC joint sprain

point tenderness, painful horizontal adduction bcs moves joint inwards

put arm at 90deg flexion, then REACH for shoulder…not just touch, put reach past
- therapist can apply overpressure if feel nothing

A

for GH dislocation or subluxation

positive: pain, apprehension in mvmnt

support shoulder and move thru 90deg of flexion
- laying down, hold arm normally

A

ulnar nerve entrapment

positive: pain, tingling thru forearm and hand, numbness

support patient’s arm, distal of elbow
- tap PROXIMALLY to the cubital tunnel w finger

elbow valgus stress test

A

stresses UCL, look for sprain/tear

positive: lig laxity or pain

patient: supinate hand, 30-40deg flexion (ask them to put arm straight, then move for them)

apply valgus force thru joint line…move joint, DO NOT rotate arm

therapist: palpate medial epicondyle

A

stresses LCL complex

positive: lig laxity or pain

palpate radial head and lateral epicondyle

A

positive = pain at medial epicondyle, no joint laxity

extend the wrist, stress wrist flexors

90 deg flexion, while supinated extend wrist and go thru full ROM extension
- palpate epicondyle at same time`

A

carpal tunnel syndrome, cubital tunnel

pain or tinging of median nerve…1st, 2nd, 3rd digits

put backs of hands tgt for 1 minute
- don’t shrug shoulders bcs will compress median nerve

can do bilateral, one at a time

finkelstein test

A

for de quervain’s tenosynovitis

positive: pain in abductor pollicis longus and extensor pollicis brevis tendons, over radial styloid process

can be passive or active test

hold wrist over table, patient opposes thumb and holds w other fingers + ulnar deviation wrist
- after this, can apply overpressure to wrist or thumb

therapist can also start by pulling fingers into ulnar deviation, no thumb involvement
