  • pregnancy. > 20 wks gestation
  • pts with increased abdominal pressure : abdominal mass ascites morbid obesity
  • certain GI pathology :esophageal stricture, SBO, gastric outlet obstruction
  • certain it’s with GERD : Active reflux, hiatal hernia
