What do you prescribe in stroke prophylaxis in AF and VTE thrombolysis?

A

Apixaban 5mg PO BDS - dose reduce in >80yo, <60kg and CrCl <133 to 2.5mg PO BDS
Rivaroxaban 20mg PO OD

A

Pabrinex = vitamin B with ascorbic acid
2 pairs TDS IV
1 pair is for prophylaxis –> insufficient for full blown WE

What is most appropriate to prescribe for a hospital patient who smokes to prevent nicotine withdrawal as REGULAR prescription?

A

Nicotine patch - SC –> dose depends on the number of cigarettes they smoke per day - look this up in BNF
Smokers >10/day - high dose patch e.g. 21mg/24hours
Medium strength patch for smokers <10/day

A

Hydrocortisone topical cream or ointment
0.5%, 1% or 2.5% - better to start low
1-2 applications per day
