Who should be offered prophylactic vaginal progesterone?
- Hx of spontaneous PTB or 2nd trimester loss OR
- Cx length ≤ 25mm
Who should be offered prophylactic cerclage?
- Cx ≤ 25mm AND
- Hx PPROM OR
- Hx cervical trauma
How is PPROM diagnosed?
- Sterile speculum examination,
- If pooling of fluid, no diagnostic test
- If no pooling of fluid, test: IGF-1 or PAMG-1
What do IGF-1 & PAMG-1 stand for?
Insulin-like growth factor 1
Placental alpha-microglubulin-1
What antibiotics should be given in PPROM?
- Erythromycin 250mg QDS
- Oral penicillin if CI, but not co-amoxiclav
How should infection be identified in PPROM?
Combination of clinical Ax & tests:
1. CRP
2. WCC
3. CTG
When is emergency cervical cerclage not offered?
- Signs of infection
- Active PV bleeding
- Uterine contractions
When is emergency cervical cerclage considered?
- 16-27+6/40
- Dilated Cx & exposed unruptured fetal membranes
- In discussion with obs & paeds consultants
