What do you perform in a neurological assessment
A GCS score
Pupils
Motor sensory assessment, checking any abnormalities
What do you perform in a respiratory assessment
Check the respiratory rate, is it fast, slow, normal
Check the rhythm, is it regular or irregular
Check the effort, is it increased, decreased
Check the skin, is it perfumed, cyanoses, blue, sweaty
Check the sounds, crackling, snoring, wheezing
Check patients speech, full sentences, phrases, singular words
What do you perform in a cardiovascular assessment
Check for pulse, is it fast, slow, normal
Check blood pressure, is it hypotensive or hypertensive
What’s the ecg read
Blood sugar level, high, low or normal
Spo2, high or low or normal
Temperature, high, low or normal
What do you check for in performing a head and toe
Head
Neck
Chest
Abdomen
Pelvis
Upper and lower limbs
Back
Toes
What is the gcs scores
Eyes, voice, motor
What is eyes, in order
- Spontaneous
- Voice
- Pain

What is voice, in order

- Obeys commands
- Localised to pain

What are we looking for in pupils
Are the reactive, are they equal?
What is the normal respiratory rate?
12-18 breaths per minute
Who many beats per minute is normal for a pulse
60-100 beats per minute
What is the terminology for blood pressure, what do they mean
Systolic pressure is less than 120
Diastolic pressure is less than 80
What is the normal number for BSL ( blood sugar levels)
4-7
What is the normal percentage of Sp02
95% and above.. anything lower is bad
Normal temp?
It varies, 36-37,, or even to 38
Capnography
Between 35-45mmHg
What are the terminology you would look for when doing a head to toe
Is there pain, deformities, bleeding, bruising, presence of fluid, swelling
History taking. What does sampled mean
Signs and symptoms, what’s going on today
Allergies, are you allergic to anything? If so what is your reaction
Medications, are you on any medication
Past medical history, has this happened before, do u suffer from another medical condition
Lasts of lasts, last meal, last bowel movement and last GP visit
Events leading up, what were you doing when it started, how long ago
Directive if old
History taking. OPQRST
Onset, when did the pain start
Provocation, does anything make the pain better or worse
Quality, what does your pain feel like
Radiation, does your pain radiate anywhere or stay in the same place
Severity, what would you rate your pain on a scale 1-10
Time, treatments and trends

Mechanism, what is the injury or presenting problem
Injuries, patient assessment and relevant history
