Primary Hyperaldosteronism (Conn's) Flashcards
(6 cards)
1
Q
what is primary hyperaldosteronism?
A
a disorder caused by autonomous production of aldosterone by the adrenal cortex (e.g. due to hyperplasia, adenoma, or carcinoma)
2
Q
what is the aetiology of primary hyperaldosteronism?
A
- adrenal adenoma
- adrenal carcinoma
- adrenal hyperplasia
- familial hyperaldosteronism
3
Q
what are the symptoms of primary hyperaldosteronism?
A
- hypertension
- hypokalaemia (e.g. weakness, fatigue, polyuria, polydipsia, paraesthesia)
- hypernatraemia
- hypervolaemia
4
Q
what are the investigations for primary hyperaldosteronism?
A
- U&Es (e.g. hypokalaemia, hypernatraemia)
- plasma aldosterone ↑
- plasma renin activity (PRA) ↓
- CT/ MRI adrenal
- bilateral adrenal vein catheterisation
- saline infusion test / oral sodium loading test (e.g. ↑ aldosterone)
5
Q
which medications should be avoided before testing for primary hyperaldosteronism?
A
- thiazide diuretics
- ACEi
- ARBs
- BBs
6
Q
what is the management of primary hyperaldosteronism?
A
- laparoscopic adrenalectomy (e.g. adrenal adenoma/carcinoma)
- spironolactone/eplerenone (e.g. BAH)
