2) Form that an entity uses to submit a plan to achieve compliance

4) Is Available to the public upon request
SNF/NF
If the providers noncompliance is isolated and does not pose a risk of more than minimal harm (S/S=A), the deficiency is cocumented on the A Form “Statement of Isolated Deficiencies Which Cause no Harm with only a Potential for minimal harm for SNFs and NFs”
The cocumentation on the Form CMS2567L would state the SNF/NF is in substantial compliance

A nursing home is in substantial compliance but has deficiencies other than the isolated deficiencies which cause no harm with only a potential for minimal harm, the deficiencies are documented on the form CMS-2567-L and no additional language regarding substantial compliance is entered on the Form CMS-2567L
