Dress Resident with a Weak Arm

* Ask them what they would like to wear. Double check with them you got the correct outfit.
* Place privacy blanket over them (do not snap or shake it) and take top sheet off
* Put socks on. Scrunch sock up, put over foot, make sure to have toe seam lined up. Guide over heal, smooth out any wrinkles
* Put pants on. Make sure tag is in the back. Insert your arm in to the pant leg and scrunch up. Place it over residents’ foot and smooth it over the heal. Pull pants up the legs as far as you can. Have them lift their hips and guide the pants all the way up.
* Put the head of the bed up.
* Assist them leaning forward and unite gown
* Tuck a corner of the blanket behind back
* Undress STRONG arm first
* Walk around bed. Undress weak arm
* Dress weak side first. Scrunch up shirt and put your arm in sleeve, hold hands (like shaking a hand) feed shirt through onto residents’ arm. Support elbow.
* Have them sit forward (they remain covered) place the shirt on the other side of resident.
* Go to the other side of the bed.
* Put the strong side in the shirt and button up shirt
* Take the privacy blanket and gown, put in laundry basket.
* Adjust clothing, throw barrier away.

Procedure

* Wash all surfaces of hand, in between fingers and thumb.
* Use other wash cloth to rinse hand
* Dry hand with towel
* Use the slanted side or the orange stick, bevel up to clean under the nails. Ask if it hurts. Wipe stick on towel in between nails.
* File any rough nails with emery board. File outer edge to middle, in one direction.
* Apply lotion to all surfaces on hand. Wipe off excess.
* Put linen in laundry. Rinse basin use paper towel to hold basin while drying them. Put soap and lotion in basin. Use towel to open drawer. Throw away barrier and supplies.

* Use other wash cloth to rinse foot.
* Dry foot with towel, use edge of towel to dry between toes.
* Apply lotion to all surfaces on foot. DO NOT USE lotion between toes. Wipe off excess.
* Put socks and shoes on.
* Put linen in laundry. Rinse basin use paper towel to hold basin while drying them. Put soap and lotion in basin. Use towel to open drawer. Throw away barrier and supplies.

* Use paper towel to turn on water, fill basin (check water) have resident check water/
* Place a privacy sheet on resident. Do not shake sheet.
* Have resident sit up to untie gown and place towel to keep sheets dry
* Remove gown on the side closest to you. Support elbow. Go on the other side and do the same.
* Don gloves
* Put wash cloths in basin
* Ring a washcloth out
* Fold wash cloth in 4’s (for the four corners method)
* Wash face. No soap. Wash closest eye inner to outer corner. Use new leaf to wash the other eye. Use another leaf to wash the remainder of the face. Use the final leaf for under the nose.
* Set wash cloth aside, Take towel and pat dry face
* Take 2nd wash cloth and apply soap
* Wash behind ear, around the chink, across the neck, across the torso, (lift blanket but keep covered until evaluator tells you to uncover) around the breast, down the upper abdomen, up the side, down the front of the arm, the hand, up the back of arm, under the arm pit. Set wash cloth aside.
* Take the 3rd wash cloth rinse out. Rise all the areas you just washed. Wash behind ear, around the chink, across the neck, across the torso, (lift blanket but keep covered until evaluator tells you to uncover) around the breast, down the upper abdomen, up the side, down the front of the arm, the hand, up the back of arm, under the arm pit. Put wash cloth back in basin.
* Put clean gown on. Put gown one closest arm support elbow. Fan out gown. Go to other side and put the other side of the gown on.
* Go back to the other side of bed have the resident move towards to so they can roll to their side away from you.
* (Evaluator will hold mannequin) Rinse wash cloth put soap on top leaf.
* Wash neck, shoulders and the back down to the waist. Set wash cloth aside.
* Take the final wash cloth rinse neck, shoulders and the back down to the waist.
* Take towel and pat dry.
* Warm lotion in hands. Start at the small of the back to the shoulders in small circles. Repeat 3 times. Wipe off excess. Tie gown
* Take towel that protects sheet. Have the resident roll back and go to the center of the bed.
* Tell resident you will be right back.
* Put linen in laundry. Rinse basin use paper towel to hold basin while drying them. Put soap and lotion in basin. Use towel to open drawer. Throw away barrier and supplies.

* Remove privacy blanket, put sheet back up and put in laundry.

* Remove bottom sheet of the side you are one. Roll it towards the resident.
* Put on new sheet and put it under the dirty sheet.
* Have resident roll back and walk to the other side of the bed.
* Have resident scoot towards you and roll to the side. (make sure they are not on the edge.)
* Take the dirty sheet off the bed. And put the new sheet on the corners. Smooth out wrinkles.
* Have resident roll back and center of bed.
* Put top sheet over the resident and take the privacy blanket off.
* Tuck the sheet at the bottom of the bed. Make hospital corners. Release any pressure off the toes.
* Change pillow case do not shake.

* Adjust arms and legs so you can roll them to the side. Make sure the privacy blanket is covering them.
* Fold a pillow in half (long ways) and place under the back. (the fold faces the back)
* Place a pillow between the legs lengthwise (between the knee and ankle) making sure the legs aren’t touching
* Place the last pillow under their top arm between the torso
* Adjust pillow under the neck, making sure they are not laying on their shoulder.
* Pull up sheet and pull off blanket. Place in dirty lien.
* Fix sheets and lower bed

* Wash the resident’s backside. Wash from the anal opening to the crease. With a new leaf wash one side of the buttocks. Then the other side with new leaf. Then one last time up the center.
* Ring out wash cloth fold into fours. No soap

Procedure

* Place barrier on the floor get supplies
* Inspect the catheter tubing.
* Remove port from protective sleeve.
* Open port over the graduate container do not let port touch anything
* Close port and place back in protective sleeve
* Fold protective barrier over graduate container, place one a flat surface (toilet lid)
* Read at eye level to the nearest mark. Note the MLs (if it is at the 200 mark it is 200 milliliters)
* Throw the barrier away, and empty the container in the TOILET rinse the graduate container and empty into the TOILET. Use paper towel to hold clean container and dry with a new one. Grab another paper towel. Use paper towel to open drawer and place inside
* Throw the paper towel away and take off gloves

Procedure
* Explain what you are going to do/ Opening
* Put privacy blanket on and lower the sheet
* Roll Chux pad and place next to resident.
* Put gloves on.
* Have resident lift their hips place chux pad under resident.
* Go on the other side of the bed and have them lift their hips and finish rolling out the rest of the chux.
* Get the bed pan out have the resident lift their hips and place the bedpan. Ask if they are comfortable.
* Put the head of the bed up.
* Hand them toilet paper
* Give resident call bell and tell them that you are going to give them privacy.
* Once they are done. Lower the bed.
* Don gloves
* Have the resident lift their hips, while holding the bed pan and chux remove both. Covering the bed pan with the chux pad.
* Empty the bed pan in the TOILET. And throw the chux pad away.
* Rinse bed pan. Pour water into the TOILET. Set the bed pan in the sink. Pick up bed pan with paper towel. Dry with other hand. One for the front and one for the back.
* Use a paper towel to put the bedpan and supplies away.
* Take gloves off
* Give resident wipes
* Take bath blanket off and put in laundry. Pull sheet up.
