Risk Factors for Thromboembolism
greater than 40 yo, prev VTE, HF, Acute resp. failure, recent major sx, IBD, MI, nephrotic syndrome, ischemic stroke, hypercoag state, vascular injury/stasis
Medications that Increase VTE Risk
Estrogen, megestrol, tamoxifen, raloxifene, general anesthesia use > 40 min
Low Risk of VTE
early ambulation or compression
Moderate Risk of VTE
Heparin 5000 units SQ q8-12h
High Risk of VTE
IPC and heparin 5000 units SQ q8h, LMWH, or fondaparinux (C/I in renal)
GI Stress Risk Factors
Mech vent for more than 48 h, coagulopathy, septic shock, head or spinal cord injury, major traum/sx, burns greater than 30% BSA, renal/liver failure, high-dose CSS
Enoxaparin PPX Dose
Dose: 40 mg daily or 30 mg q12h: CrCl less than 30: 30 mg daily
Famotidine Dose
Dose: 20 mg q12h, or 40 mg daily; CrCl less than 50: half daily dose
Pantoprazole Dose
40 mg daily or 12 hr
