T or F Prostate cancer is the most hormone sensitive of all cancer.
T
What will a gain in DHEAS stability lead to?
DHT tumor
What drug category causes disease flare and how are they avoided?
GnRH agonist, 2-4 wks of AR blockers
Can GnRH agonists be given to a pregnant woman?
Cat X, so no
ADE of GnRH agonists
CV effects: edema, HTN, rarely MI (leuprolide): headache, fatigue, depression, seizures (histrelin) suicidal ideation (histrelin)
Hypo-adrenergic state: decreased BMD, elevated TG, weight gain, DM, reduced libido, sexual dysfunction, gynecomastia
Rarely hepatotoxicity
Aches and pains
MOA of degarelix
reversible GnRH receptor antagonist, castrate testosterone levels within 3 days with no disease flare,
ADE of degarelix
Elevated hepatic enzyme level and QT prolongation
hot sweats, injection site reaction, weight gain, hypertension, athralgia, chills, fatigue, importence
MOA of estramustine
bind EBP on prostate and inhibits microtubules, promoting dis-assembly and G2/M arrest produce DNA strand breakage. Delivers alkylating moiety to prostate.
ADE of estramustine
GI upset, gynecomastia, mastalgia, impotence, increased CV risk (edema, TE, MI, PE, stroke) hepatic enzyme and hyperbilirubinemia
Can you give androgen receptor blockers to a pregnant woman?
No- they are teratoges except nilutamide
