Psche--MSK Flashcards
(41 cards)
Pain over AC, pain with lifting arm
Patient supports arm in ADDUCTION
TTP over AC joint
I to VI degrees
Special test: Cross body Adduction
II-VI obvious deformity
Acromioclavicular Injury
Result from falling on shoulder or being struck over clavicle
Pain with shoulder motion, erythema, edema, TTP
bony deformity
SHOULDER DROOP
Grinding noted on ROM
Special test: Cross body POSITIVE
Clavicle fracture
Gradual onset of anterior/lateral shoulder pain exacerbated by overhead activity
night pain and difficult sleeping on affected side
Palpation: tenderness over greater or lesser tuberosity, tenderness over bicipital groove
pain worse 90-120degree of abduction and when lowering arm
Special test: NEERS/ Hawkins
Impingement syndrome
Chronic shoulder pain for several months
specific injury that triggered pain
night pain and difficulty sleeping on affected side
complaints of weakness catching/grating with OVERHEAD activities
Shoulder may appear sunken
Special Test: POS Drop arm/empty can
Rotator cuff Tear
From mild to rupture
rock climbers, weight lifters
aggravated by lifting/pulling overhead activity
anterior shoulder pain that radiates distally down the arm over the bicep muscle
occurs along the long head of bicep tendon
Special Test: POS Speeds/Yergason
Bicep tendinopathy
Trauma or overuse
Anterior: sensation slipping out supports arm in neutral position
Posterior: sensation of force posteriorly directed, ADDUCTED and INTERNAL ROTATION
Special Test: Sulcus
Apprehension
JERK
Shoulder dislocation/instability
Falling back on outstretch arm grab object while falling lift/forceful throw lift heavy object fas clicling/clunking, swelling paresthesia
Special Test: Speeds
Labrum/SLAP Tear
Gradual onset with global limitation both active/passive
diffuse/sever/disabling/ increasing in stiffness and loss of ROM
FROZEN SHOULDER
Adhesive Capsulitis
pain with resisted extension of wrist
turning a screw driver- excessive typing TTP over extensor
Tennis Elbow
lateral epicondylitis
Over use of flexor and pronator muscles
pain with resisted flexion of wrist
gradual onset, bowling, wt. lifting, manual labor
golfers/bowlers elbow
Medial epicondylitis
Gradual/sudden onset of welling in elbow possible infection
limited flexion and extension
Olecranon bursitis
cubital/numbness digits 4+5 may radiate to shoulder and nec
vibration sensations in 4-5 will be diminished
Special Test: Tinels
Nerve compression syndrome
Ulnar excessive overhead throwing motion
Acute onset patient will describe a “pop” while throwing
gradual onset with progressive medial elbow pain with valgus stress
Special Test: POS Valgus stress test
Ligamentous Injury
most common in children Resulting from FOOSH: falling on outstretched hand extreme, pain, swelling, inability to bend elbow visual deformity all motions will be limited
Elbow Dislocation
Middle aged or pregnant women
Condition caused by reduce zie/space of carpal tunnel resulting in median nerve entrapment
numbness and tingling 1-2-3 digits.
pain and paresthesia or numbness of the media distribution
worse at night.
PATIENT RUBS HANDS TO GET SIRCULATION BACK
Frequently drop objects
Special test: phalen/Tinel
Carpal Tunnel Syndrome
precipitated by repetitive use of thumb
Pain at the radial aspect of the wrist exacerbated by movement of the thumb or wrist
occasional edema
de Quervain Tenosynovitis
Rupture of the flexor digitorum profundus tendon from its distal attachment common in caontact sports overlooked as a "jammed" finger FLEXED DIP is FORCEFULLY HYPEREXTENDED INABILITY TO ACTIVELY FLEX DIP JOINT ACUTE PAIN WITH SWELLING
Jersey finger
finger- Injury to extensor
rupture, laceration or avulsion of insertion of the extensor tendon and base of distal phalanx
direct blow to finger causing sudden forced flexion of dip
pain at dip with erythema, edema, ecchymosis, deformity
Mallet Finger
lateral hip pain with tenderness
pain may radiate distally to knee or ankle or proximally to buttocks
pain worse when going from sit to stand
Pain made better by warming up may return after 30-1hr walking
unable to lie on affected side
Trochanteric Bursitis
Sudden pain and giving way of the knee from a twisting or hyperextension type injury
Possible pop sound
Unable to continue activity to pain or instability
Moderate to severe effusion and possible hemarthrosis (bleeding into joint)
Special Test: Anterior drawer - Lachman’s test
ACL Tear
Dash board injury
pure hyperflexion of knee
fall onto a flexed knee with the foot in plantar flexion
moderate to severe effusion and possible hemarthrosis. muscle guarding
Special tests: POS posterior drawer
SAG TEST
PCL tear
knee tear from valgus force
localized ecchymosis and small effusion
Special test: valgus stress
MCL
knee tear from varus force
localized ecchymosis and small effusion
Special test: varus stress
LCL
High energy trauma/MVA
Hip
limb is short, hip flexed ADDUCTED AND INTERNALLY ROTATED
severe pain unable to more limb
Posterior- Hip dislocation
