Anxiety is normal however in extreme cases it leads to overly stressing about unimportant things

Planning & Organising scenario

A

Situation - the situation you had to deal with
* Working on Glencree acute ward in Wicklow
* Patients divvied out based on where they are from
* Preceptor and I given largest group of patients and split into 4/4
* Busy day as MDT day for that group of patients
* Preceptor had to go home sick
* Resulted in me having full group of 8 patients/MDT for each
* Later in the evening there was an admission to the ward which I also wanted to get involved in

Task – the task you were given to do
* Normal tasks of the role – Checking obs/talking to patients, planning for and participating in MDT.

Action - the action you took
* Initially sat down and made a to-do list of necessary and other tasks
* When preceptor went home sick I sat down again and reassessed what I needed to do
* Had a discussion with all the patients in advance of the MDT
* Attended MDTs.
* Later in the day when I was organising my notes for the day, heard that there would be a new admission for one of the other teams and felt it would be a prime learning opportunity
* Had a discussion with CNM on the ward regarding my caseload and that I would like to take part in admission.
* CNM offered to take a few of the patients notes off my hands and allow me to take part in the admission
* Had just given handover to CNM of patient details.

Result - what happened as a result of your action and what you learned from the experience
* Patients received decent standard of care
* MDTs were all attended
* I found that having organised my day in advance was very beneficial however It being an acute ward I needed to be willing to understand that plans may change at any moment and I will need to reprioritise accordingly

Building and Maintaining Relationships scenario

A

Situation – the situation you had to deal with
* Working on a busy acute ward
* Patient noticeably agitated before an MDT.
* Had a discussion and they reported being nervous for the MDT due to the big groups that are often in them and their inability to remember topics they want to cover when in there

Task – the task you were given to do
* As this patient was my responsibility I felt I should provide the with support
* I was due to go into the MDT with them as I was in charge of that team for the day.

Action
* Firstly I informed the patient that I will be going into the meeting with them so if there is anything I can do to support them.
* I offered to discuss what topics they would like to cover in the meeting and got them a pen and paper to write down areas they wanted to discuss.
* I attended the meeting with the service user.

Result
* The patient was observed to be more relaxed as well as able to approach the necessary topics for the MDT.
* They thanked me afterwards
