What happens to the epithelium straight after treatment?
Bacterial remnants are washed out of the pocket by blood and gingival fluid.
Within a few hours an acute inflammatory reaction occurs in the soft tissue pocket wall.
What happens within 2 days of receiving treatment?
The remaining pocket epithelium, proliferates and the pocket wall fully heals within 2 days. The new junctional epithelium reattaches to the tooth via hemidesmosones.
What occurs after 5 days post treatment?
Epithelial reattachment starts apically and progresses coronally.
What occurs after 14 days post treatment?
Epethelial attachment is complete and a new gingival sulcus is formed near the crest of the gingiva.
What occurs at week 3 post treatment
The formation of functional collagen that replaces granulation tissue
What occurs during the 3-6 week period post treatment
Probing depths continue to reduce until collagen regeneration is complete. Maximum probing resistance now occurs.
Most of the reduction in probing depths/gain in clinical attachment takes place within 6 weeks
What occurs during the later stages
Maturation of the connective tissue component may continue for several months
Infra bony pockets may have some bone fill

What effects occur after PMPR
Decrease in gingival inflammation
Reduction in probing depths
Gain in gingival attachment
Changes in subgingival microbial flora
Decreased tooth mobility
Possible in-fil of infra-bony defects
Change in attachment due to long junctional epithelium formation and improved tissue tone
Deep pockets resolve quickest
What must you do before starting treatment
Gain consent (GDC standard 3)
Gather information (medical history etc.)
Risk assessments such as:
BPE
6PPC
caries
Challacombe
BEWE
Soft tissue exam
What area would you start treatment on?
Areas of 4* or highest area …
