(13 cards)
Give 3 clinical signs of respiratory distress
- Pursed lip breathing
- Nasal flaring
- Intercostal recession
What may be heard on auscultation of a patient with COPD?

What are the differential diagnoses for COPD?
- Cardiac failure with pulmonary oedema
- Extrinsic allergic alveolitis
- Interstitial lung disease
Outline the components of the MRC dyspnoea scale
Grade 1 - Not trouble by breathlessness except with strenuous exercise
Grade 2 - Troubled with SOB when hurrying on level ground or walking up a slight hill
Grade 3 - Walks slower than people of the same age on level ground due to breathlessness
Grade 4 - Stops for breath after walks about 100m
Grade 5 - Too breathless to leave the house/ breathless when dressing and undressing
Which medications are known to interact with theophylline and increase the risk of toxicity as a result
Macrolide and quinolone antibiotics
Suggest 5 clinical signs of theophylline toxicity
- Tremor
- Vomiting and diarrhoea
- Confusion
- Seizures
- Cardiac arrhythmias
What would you offer to complete a respiratory examination in which you suspect a diagnoses of lung cancer?
- Oxygen saturations and temperature
- Measure peak flow
- Examine sputum pot
- Check for signs of extra-pulmonary involvement by palpating the spine and liver and performing a neurological exam of the arms, legs and cranial nerves
What are the typical features of squamous cell ling cancer?
- Central cavitating lesion
- Personal smoking Hx
- Paraneoplastic syndromes e.g. hyperthyroidism (ectopic TSH) and hypercalcemia (ectopic PTH)
What are the typical features of adenocarcinoma of the lungs?
- Peripheral solid lesion
- Often patient will NOT have a smoking history
- Seen in association with asbestos exposure
What is the characteristic clinical feature of bronchoalveolar carcinoma?
Production of copius amounts of sputum

Endocrine cells - Kulchitsky cells
Recall 3 neoplastic phenomena associated with small cell lung cancer
- SIADH
- Ectopic ACTH
