A
  • Rheumatoid nodules (on extensor surfaces & pressure points)
  • Ocular (episcleritis, scleritis & scleromalacia perforans)
  • Pulmonary (pleural disease, ILD, bronchiolitis obliterans, pulmonary nodules)
  • Cardiac (pericarditis, myocarditis, conduction defects)
  • Hematologic (IDA, Felty syndrome [splenomegaly, neutropenia & leg ulcers], high or low platelets, Non-Hodgkin lymphoma)
  • Neurologic (atlantoaxial instability/subluxation), peripheral neuropathy, ischemia associated with vasculitis
  • Rheumatoid vasculitis (leukocytoclastic, cutaneous ulcers)

  • epi: 20-60 yo; AA at increased risk, disease onset 10 years earlier
  • path: multisystem noncaseating granulomatous disorder (T helper & inflammatory cells) of unknown etiology
  • pres: fever, fatigue, weight loss, polyarthritis, cough, hemoptysis, DOE, Bell palsy, peripheral neuropathy, subQ nodules, lupus pernio, CHF, dysrythmias, uveitis, conjunctivitis, LAD
