What are systemic symptoms you would see in a patient with rheumatoid arthritis?
Bouchard nodes, swan neck, deformity, boutonnieres
Pain and stiffness in the morning that last longer than 30 minutes
Fever, weight, loss, fatigue
What is the treatment for rheumatoid arthritis?
DMARDS: methotrexate
What is methotrexate notorious for causing?
Folic acid deficiency and bone marrow suppression, order a CBC
What are symptoms of osteoarthritis?
Joint pain and stiffness in the morning that resolves in less than 30 minutes
Heberden nodes (DIP) and Bouchard nodes (PIP)
What would you see on an x-ray with someone who has osteoarthritis?
Joint space narrowing
What is the treatment for osteoarthritis?
NSAID’s and exercise
Where is the heberden nodes located?
DIP: distal interphalgeal joint, the joint closest to the tip of the finger
Where is the bouchard nodes located?
PIP: proximal interphalgeal joint, usually in middle of the finger
What are bouintneres in rheumatoid arthritis?
Makes your finger bend “flexed at proximal interphlangeal joint”
What laboratory test is most specific for rheumatoid arthritis?
Anti-CCP Antibodies (Anti-cyclic citrullinated peptide antibodies )
ESR and C- reactive protein are not specific to RA.
Rheumatoid-Factor is specific to RA but not as specific as Anti-CCP
