parents (step and parents in law)
brothers/sisters (half siblings and brothers and sisters in law)
aunts/uncles
grandparents
grandchildren
other family that permanently reside with employee

*special consideration can be given to any other person who has had a close, long-standing, personal relationship with the employee, where such a person acted similar to, and/or stood in the place of, a mother, father, brother, sister, or grandparent of the employee.
