What are in the General Survey of ROS?
Usual weight, recent weight change, fatigue, fever, sleep patterns or insomnia
Skin ROS
Rashes, lumps, lesions, itching, dryness, color change, changes in hairs, nails.
Head ROS
Headaches, head injury.
Eyes ROS
Vision, glasses or contact lenses, last eye examination, pain, redness, excessive tearing, double vision, spots, specks, flashing lights, glaucoma (damage to the eye’s optic nerve), cataracts (clouding of the lens of the eye, normally clear)
Ears ROS
Hearing acuity (hearing loss overtime), tinnitus (ringing/buzzing of the ear), vertigo, earaches, infection, discharge
Nose/Sinuses ROS
Frequent colds, nasal stuffiness, discharge, itching, hay fever, nosebleeds, sinus trouble.
Mouth/Throat
Condition of the teeth, bleeding gums, last dental exam, sore tongue, frequent sore throats, hoarseness.
Neck ROS
Lumps in the neck, swollen glands, goiter (enlarge thyroid), pain, stiffness in the neck
Breasts/Axilla
Lumps, pain or discomfort, nipple discharge, self-examination
Respiratory ROS
Cough, sputum (color, quantity), displeasure, hemoptysis (coughing up blood), wheezing, asthma, bronchitis, emphysema (block airflow and make it difficult to breathe), pneumonia (inflamed air sacs, may contain fluid), tuberculosis, pleurisy (inflamed tissues that line the lungs and chest cavity), last chest x-Ray.
Cardiac ROS
heart failure, high blood pressure, rheumatic fever, heart murmurs, chest pain or discomfort, palpitations (strong or irregular heartbeat due to agitation, exertion, or illness), orthopnea (shortness of breath occurs when lying flat causing patient to sit up), paroxysmal nocturnal dyspnea (wake up at night, frighten because you can’t breathe), edema (excess water collecting in cavities or tissues of the body), past EKGs or other tests.
Gastrointestinal ROS
Trouble swallowing, heartburn, change in appetite, nausea, vomiting, regurgitation, vomiting of blood, indigestion, frequency of bowel movements, color and size of stools, change in bowel habits, rectal bleeding, black tarry stools, hemorrhoids, constipation, diarrhea, abdominal pain, food intolerance, excessive belching or passing of gas, jaundice, liver or gallbladder problems, hepatitis.
Genitourinary ROS
Frequency of urination, polyuria (freq. production of dilute urination, diabetes), nocturia (urination at night), burning or pain in urination, hematuria (blood in urine), urgency, reduced caliber or force of stream, hesitancy, incontinence (no control you pee all over yourself), urinary retention, stones
Peripheral vascular
Intermittent claudication (cramping in pain that is induced by exercise) caused by obstruction of the arteries, leg cramps, varicose veins, clots in the veins, Raynaud’s disease (some areas of body feel numb and cool in certain circumstances).
Musculoskeletal ROS
Muscle or joint pain, stiffness, arthritis, gout, backache (if any describe the location and associated symptoms- swelling redness, pain tenderness, stiffness, weakness, limitation of motion or activity).
Neurologic ROS
Fainting, blackouts, seizures, weakness, paralysis, numbness, tingling, tremors, or other involuntary movements, change in attention span.
Psychiatric ROS
Nervousness, tension, mood changes (including depression), any suicidal ideation or attempts, changes in memory.
Hematologic ROS
Anemia, easy bruising or bleeding, past transfusions/transfusion reactions.
Endocrine ROS
Thyroid trouble, heat or cold intolerance, excessive sweating, diabetes, excessive thirst or hunger, polyuria (produce a large volume of dilute urine).
Male Genitourinary Added ROS
Hernias, penile discharge or lesions, testicular pain or masses, STIs and their treatments, exposure to HIV, precautions against HIV and STIs.
Sexual interest, orientation, function, satisfaction/problems, method of contraception.
Female Genitourinary ROS
Age at menarche, regularity, frequency, and duration of menses, amount of bleeding, bleeding between period or after intercourse, Last me trial period (LMP), dymenorrhea (painful during mense), premenstrual tension.
Age at menopause, menopausal symptoms, post menopausal bleeding, vaginal discharge, itching, lesions, lumps STIs, and their treatments, exposure to HIV, precautions against HIV and STIs.
Number of pregnancies, number and type of deliveries, number of abortions (spontaneous and induce), complications of pregnancy, methods of contraceptions.
Sexual interest, orientation, function, satisfaction,/ problems clouding dypareunia (difficult or painful sexual intercourse
