Why treat patient in the home instead of hospital
- convenience
- -> less traveling
- less cost
- -> avoid ancillary fee
- psychologically beneficial
- ->comfort with family
- ->more autonomy
- ->less likely to acquire nosocomial infection
What is the benefits of home care system?
Improves:
- motor coordination
- reaction time
- sleep quality
- exercise capacity, endurance
- decrease shortness of breath
- increase quality of life
What medical gas delivery system can we supply at home?
- liquid o2 system
- concentrator
- cylinder
Who funds O2 at home?
Assistive Devices Program (ADP) funds 100% >64 yr old
unless
–> qualified for other social program
If under 65 , will be funded at 100% in certain circumstances
***funding rules and qualification change constantly every year
Advantages of liquid O2 system ?
1) requires no power
2) medium to high flow system
3) portable with stroller
**good for patient with good mobility and physical capacity
Disadvantage of liquid o2 system?
1) filling required, needs coordination
2) costly to supplier
What are the safety issue of Liquid o2 system?
- venting during storage (o2 in environment is hazardous )
- risk of frost bite
- risk of valve staying open
What are the advantage of a concentrators?
- convenient
- no filling required
- easy to operate
- incorporated alarms
- routine maintenance required by a technician
- can be portable
**good for nocturnal, long term, palliative use
What is the disadvantage of concentrators?
- a lower flow system
- noisy
- electrical power source required (AC/DC)
- BACK UP SYSTEM –> in case of power shortages
- produce significant amount of heat
What does concentrator maintenance include?
1) assessing O2 concentration (fio2)
2) wash /replace intake filters
3) check flow rates
4) Check alarms
What is advantages of cylinder?
- most economical with low flow
- can be low, medium, high flow
- ideal for back up and for infants at low flow
**good for long term storage/infrequent use
What is the disadvantages of cylinders?
- frequent replacement
- heavy unless it is aluminum
- security and storage consideration
