What is PPROM?

A

ROM occurring before 37 wks GA

baby isn’t ready, mom isn’t ready either

What tests are used to confirm ROM?

A

POOLING: collection of fluid in vaginal vault

NITRAZINE: blue color change due to the alkalinity of amniotic fluid

FERNING: estrogens causing crystallization of salts in amniotic fluid when it dries (Note: do not sample fluid directly from cervix)

Oligohydramnios (especially if fluid was normal)

INDIGO CARMINE Dye injection via Amniocentesis (look for leakage of fluid from cervix onto tampon)

AMNISURE: identifies placental alpha-macroglobulin-1 via immunoassay which is sensitive and specific
