Why did Chernobyl happen and how could it have been avoided?

A

It happened due to a pressure and power buildup in the lower parts of the reactor, it also happened due to the reactor being built poorly.
It could have been avoided if the building of the reactor had not been rushed and if the director had not had such a superiority complex.
