Spinal white matter: sensory pathways

Dorsal and Lateral Column
1. Dorsal column
2. Spinocerebellar
3. Spinothalamic
4. Spinoreticular

2

Spinal white matter: motor pathways

Anterior and Lateral Column
1. Corticospinal
2. Rubrospinal
3. Reticulospinal
4. Vestibulospinal
5. Tectospinal

3

Dorsal column pathway

Sensory:
Conscious touch and proprioception

1. Touch/proprioception info from limbs enters SC
2. Ascends in dorsal column to ipsilateral medulla
3. Synapse in medulla
4. Crosses spinal cord and ascends in medial lemniscus to contralateral thalamus
5. Synapse in thalamus
6. Ascends to cortex

4

Spinocerebellar pathway

Sensory:
Unconscious proprioception

5

Spinothalamic pathway

Sensory:
Conscious pain and temperature

1. Pain/temperature information from limbs enters SC and synapses in dorsal horn
2. Crosses to contralateral SC
3. Ascends in spinothalamic tract
4. Synapse in thalamus
5. Ascends to cortex

6

Spinoreticular pathway

Sensory:
Alerting, arousal

7

Corticospinal tract

Motor:
Voluntary movement and fine motor control

1. Cell body in motor cortex
2. Crosses in medullary pyramid
3. Descends in lateral column
4. Synapses w/ alpha motor neuron

8

Rubrospinal tract

Motor:
Gross motor movements

1. Cell body in red nucleus
2. Crosses immediately
3. Synapses w/ alpha motor neuron

9

Reticulospinal tract

Motor:
Trunk movements, postural control

10

Vestibulospinal tract

Motor:
Trunk movements, postural control

11

Tectospinal tract

Motor:
Reflexive movement of the head

12

Upper Motor Neuron (UMN)

Clinical signs: Paresis, spasticity, hyperreflexia, hypertonia
Diseases: Stroke, TBI, SCI, MS, PD

13

Lower Motor Neuron (LMN)

Clinical signs: Atrophy, flaccid paralysis, loss of reflexes, fibrillations/fasciculations
Diseases: Polio, Peripheral Nerve Injury, Guillian Barre Syndrome

14

Amyotrophic Lateral Sclerosis (ALS)

AKA Lou Gehrig's Disease
Both an UMN and LMN disease
