also shall keep billing records made to patients and third party carriers

what does the tx record need to have
- date of tx
- CC
- hx
- progress notes
- orders for tests or consultations
- informed consent documentation
- exam findings
- referral or dx by physician (name of dr, )
- recommendations made for other health care products or meditation
how do you make chart corrections

how shall a chart note made and maintained on a personal computer be prepared or kept
- it must have 2 forms of identification (name and record number)
- entry shall contain date of service, full printed name of provider, chart must be finalized with a confidentail personal code (cpc) and date
transcipted notes must
be dated and identified as preliminary
computer system must have what
internal permanent date and time stamp for all entries
and once chart note signed cannot be changed
who gets access to the chart
physician OR representative of the board, the attorney general or the divison of cnsumer affairs no later than 10 days after a request is made
to the patient( or pt representative) within 30 days of the request
how can a chart note be given to a patient
the actual chart info as, tx record, billing record, test results, subjective info OR a summary of that info
for record copies given what can we charge
no more than $1/page or $100 for the entire record whichever is less. if record less than 10 pages can ask for $10. q
if subpoenaed, we release records where and how
attorney general or board, such records shall be originals, unedited with full patient names.
