Orientation

  • What is the year, season, date, day, moth?
  • Where are we? state, county, town, hospital, floor

Registration
-Name 3 objects: 1 second to say each. Then ask the patient all 3 after you have said it. Then repeat until he learns all 3. Count # trials and record

Attention and Calculation
-Spell the word “world” backward

Recall
-Ask for the 3 objects repeated above.

Language

  • Point to a pen, and watch and ask the patient to name it
  • Repeat the following “No if’s, and’s or but’s”
  • Follow a 3-stage command “ take a paper in your right hand, fold it in half, and put it on the floor”
  • Read and obey: Close your eyes, have them write a sentence or have them copy a design (2 intersecting pentagrams)
