SEPSIS - Adult

A

Patient is generally unwell with suspected infection and a temperature of less than 36 or above 38.3
-Heart Rate, Respiratory rate, ECG and SpO2 monitoring
-Is there abnormal physiology, do you know the source of the injection
-Is the patient at risk
-If they are not at risk, monitor clinical condition, re-evaluate for possible sepsis if clinically indicated, transport
-If they are at risk, “Give 3” is clinically indicated (ALS MUST GIVE 2 & 3 AS IV)
-oxygen therapy
-Is there signs of hypoperfusion
-If there are no signs of hypo perfusion continue to monitor clinical condition, re-evaluate for possible sepsis if clinically indicated and transport
-If there are signs of hypo perfusion, request ALS, ALS must treat, transport

-IF TEMPERATURE IS ABOVE 38.3 CONSIDER 1G OF PARACETAMOL PO
-GIVE 3
1. O2 TITRATE TO SATS ABOVE 94% (88 TO 92% FOR CHRONIC LUNG CONDITIONS EG COPD)

-EVIDENCE OF AT RISK CRITERIA (ANY 1 OF 3)
1. ANY 1 CLINICAL SIGN OF ACUTE ORGAN DYSFUNCTION
2. AT RISK OF NEUTROPENIA (BONE MARROW FAILURE, AUTOIMMUNE DISORDER, TREATMENT INCLUDING BUT NOT LIMITED TO CHEMO/RADIOTHERAPY), NOTE THAT THESE PATENTS MAY PRESENT WITHOUT A FEVER
3. 2 OR MORE SIRS CRITERIA PLUS 1 OR MORE CO-MORBIDITY

-SIGNS OF SYSTEMIC INFLAMMATORY RESPONSE SYNDROME (SIRS)
-TEMPERATURE LESS THAN 36 (COLD SEPSIS) OR ABOVE 38.3
-HEART RATE ABOVE 90 BPM
-RESPIRATORY RATE ABOVE 20
-ACUTELY CONFUSED
-BLOOD GLUCOSE LEVELS ABOVE 7.7 (NON DIABETIC)
-IF THE PATIENT HAS TWO OR MORE OF THESE SIGNS, CONSIDER SIRS

-HIGH CONSEQUENCE INFECTIOUS DISEASE (HCID) ENSURE APPROPRIATE PPE IS WORN
-LONG SLEEVE GOWN
-FACEMASK
-EYE PROTECTION

PRE-ALERT THE ED IF
-SEPSIS
-SEPTIC SHOCK
-MENINGITIS SUSPECTED
-AT RISK OF NEUTROPENIA

IF SIRS PLUS INFECTION ADVISE TRIAGE NURSE
