What are the key parameters to measure in suspected sepsis?
- Blood pressure
- HR
- Urine output
- Serum lactate
- Oxygen sats
- Temperature
What is the definition of septic shock
Sepsis with hypotension, unresponsive to fluid resuscitation, with lactate > 4mmol
What are the features of SIRS?
Temperature >38 or <36 degreesC
Heart rate >90
Respiratory rate >20
White cell count >12 or <4 x10^9/L
Management of sepsis?
“ABC”
Airway
Breathing
Circulation
“sepsis six”
Take bloods (FBC, U&E, LFT, CRP, lactate) Take blood cultures Administer oxygen if required Administer IV antibiotics Administer IV fluid resuscitation Monitor urine output
Other investigations:
Imaging e.g chest xray, echocardiogram, abdominal ultrasound
Viral PCR for e.g. influenza
Urinalysis +/- culture
Lumbar puncture
Ongoing management depends on the source and severity:
IV antibiotic therapy
Surgical removal of source of infection
Critical care involvement
Signs and symptoms of sepsis?
Fever, sweats or chills Breathlessness Headache Nausea and vomiting Diarrhoea
Observations:
- Tachycardia
- Hypotension
- Pyrexia
- Peripherally vasodilated
- Hypoxia
- Tachypnoea
