Fall A

45-årig kvinna inkommer på larm till akutmottagningen medvetandesänkt och hypoton. Medföljande make berättar att hon insjuknade hastigt 6 timmar tidigare med frossa och hög feber. Blivit allt slöare sista timmen. Är tidigare frisk. Äter inga mediciner.

Status AT: medvetandesänkt, kall och marmorerad perifert. Peteckier på armar och ben. Systoliskt blodtryck 65. Hjärtfrekvens 145. Ej nackstyv.

Handläggning initialt?

A

Klart: svår sepsis (hypotension + sepsis [tackykardi, feber])

Prel diagnos: meningit (peteckierna talar för det)

Diff: lunginflammation, urosepsis, encefalit, pyelonefrit

ABCDE (Airways, Breathing, Circulation, Disability, Exposure/Environment) –>

Syre (kanske 5 - 15 L - så att pt stiger?)

Vätska: 2 påsar Ringer (1 L direkt, 1 L senare)

Odling: blod (2 set [anaerob + aerob], olika lokaler), sår, nasofarynx, urin (+ ev urin-pneumokock/legionella-AG)

Blodgas: laktat, saturation - följ, katet - minst 2 infarter

A/b: cefotaxim 3gx4 + ampicillin 3g*4 + steroider (vanligen betametason - dämpar det inflammatoriska svaret vid sönderfall av bakterier orsakat av a/b) (som för meningit)

Generellt: Samhällsförvärvad infektion utan kända riskfaktorer och med okänt fokus - bakterier som måste täckas: S aureus, pneumokocker, beta- hemolyserande streptokocker, E coli, Klebsiella och andra enterobacteriacae

Dosering:

  • Vid svår sepsis och septisk chock kan volymen av den interstitiella vätskan mer än fördubblas och göra att antibiotikakoncentrationerna vid normal dosering blir för låga. Distributionsvolymen för vattenlösliga antibiotika såsom betalaktamer är 2‐3 gånger högre än hos friska.
  • Betalaktamer (både cefotaxim - cefalosporin och ampicillin - penicillin är det) ges därför i en extrados efter halva dosintervallet mellan första och andra dosen – t ex vid 0,4,8,16 (och 24 timmar) det första dygnet.

GCS > 8 (avvakta med LP vid medvetslöshet = GCS ≤ 8; om fokal neurologi - DT först för att utesluta hjärnabscess) –> LP –> klar/serös el grumlig:

  • < 1000 celler serös, > 1000 celler purulent
  • Poly –> bakteriell, Mono - viral

Petechier + blododling –> meningokocker

OM pneumokocker: PcG = bensylPC (kåvepenin)

Skulle kunna vara pyelonefrit E coli

Kan man få sepsis av virus?

Blodstatus, koag-status, lever, CRP, TPK. Kan ta mobil rtg.

Agens: meningokocker, E coli, pneumokocker (Strep pneumoniae), S aureus

  • måste täcka för dessa –> piptaz eller cefotaxim alt meropenem
  • Meningokock - Neisseria meningtidis: G-, diplokock, aerob

Neisseria är ett släkte av bakterier. De är aeroba gramnegativa kocker, oftast arrangerade två och två, och kallas då diplokocker. Alla arter av Neisseria är oxidaspositiva, och de flesta producerar även katalas. Inom släktet Neisseria finns ett tiotal arter varav två är strikt humanpatogena N. meningitidis, “meningokocker” som orsakar meningit, och N. gonorrhoeae, “gonokocker” som är orsaken till gonorré. De andra arterna av Neisseria finns ofta som en del av normalfloran i näsa och svalg. Släktet Neisseria har fått sitt namn från den tyske läkaren Albert Neissersom var den förste som beskrev N. gonorrhoeae.

Meningokockinfektioner drabbar framför allt barn, tonåringar och yngre vuxna. Senare i livet är den mycket sällsynt, vilket beror på förvärvad immunitet. Bakterien påvisas ofta i näsa och svalg, även från helt friska personer. Bärarskap av meningokocker kan förekomma i uppemot 10 procent av befolkningen. I omgivningen kring ett kliniskt fall kan ännu högre frekvens ses. Invasiv sjukdom drabbar företrädesvis de som är nykoloniserade med meningokocker, inte kroniska bärare. Vårdnivå: IVA, inotropt stöd (se not längst ned).

BEHANDLING (Internetmedicin)
Vid sepsis och meningit skall intravenös antibiotika omedelbart ges. Innan meningokockdiagnosen är säkerställd ges intravenös antibiotikabehandling med brett spektrum, t ex cefotaxim (Claforan, Cefotaxim) + ampicillin (Doktacillin) alternativt meropenem (Meronem) vid meningit och cefotaxim alternativt bensylpenicillin + aminoglykosid (ex Nebcina) vid sepsis.
När meningokockdiagnosen är säkerställd ges bensylpenicillin i.v. i dos 4 g x 3-4 till vuxen vid meningit och i.v. 3 g x 3-4 till vuxen vid sepsis. Behandlingstid: 5-7 dagar.

Invasiv meningokockinfektion är en anmälningspliktig sjukdom och inträffade fall anmäls till smittskyddsläkaren i landstinget och Folkhälsomyndigheten. Tidig telefonkontakt är av yttersta vikt. Odlingspositiva enbart från svalg eller nasopharynx anmäls inte.

  • WIKI: Pneumokockinfektioner är i regel lätta att behandla med antibiotika, penicillin, och det är ovanligt att i övrigt friska personer dör av denna infektion, även om det händer i ett par fall per år i Sverige. Vissa pneumokockinfektioner är resistenta, eller har en nedsatt känslighet mot penicillin. Trots detta kan höga doser bota även dessa typer. En del kan även behandlas med cefotaxim intravenöst, meropenem eller vankomycin.
  • Enklast diagnostiseras E. coli-infektioner genom att göra en bakteriologisk odling från den infekterade lokalen, till exempel urin eller blod. Eftersom urinvägsinfektioner oftast orsakas av E. coli behöver läkaren ofta inte företa odling av urin, utan kan behandla infektionen med antibiotika ändå.

Infektioner som orsakas av E. coli är behandlingsbara med ett flertal olika antibiotikatyper som pivmecillinam, ampicillin_, cefalosporiner, trimetoprim, kinoloner_. Mot ampicillin, som är ett slags penicillin, föreligger dock relativt ofta resistens, varför just detta antibiotikum används relativt sällan numera.

  • Medscape: Because community-associated methicillin-resistant Staphylococcus aureus (CA-MRSA) causes more than one half of all staphylococcal infections in most communities, empiric therapy with penicillins or cephalosporins may be inadequate.[9] Some experts recommend combination therapy with a penicillinase-resistant penicillin or cephalosporin (in case the organism is methicillin-sensitive S aureus [MSSA])[10] and clindamycin or a quinolone. Others suggest use of clindamycin, trimethoprim-sulfamethoxazole (TMP-SMX), rifampin, doxycycline, or a quinolone**. Finally, because of concerns about induction of resistance, some recommend using **TMP-SMX and rifampin in combination, rather than singly. As data accumulate, clindamycin may become the preferred outpatient antibiotic therapy (compared with TMP-SMX) in regions with a relatively low incidence of clindamycin resistance.[11] Recently, treatment guidelines have been published.

Sepsis o endokardit

Sepsis –> svår sepsis –> septisk chock (BT < 90, perifert kall)

Laktat igen efter 30 min (?) –> trolig septisk chock.

WIKI: An inotrope (etymology and pronunciation) is an agent that alters the force or energy of muscular contractions. Negatively inotropic agents weaken the force of muscular contractions. Positively inotropic agents increase the strength of muscular contraction.

The term inotropic state is most commonly used in reference to various drugs that affect the strength of contraction of heart muscle (myocardial contractility). However, it can also refer to pathological conditions. For example, enlarged heart muscle (ventricular hypertrophy) can increase inotropic state, whereas dead heart muscle (myocardial infarction) can decrease it. Positive inotropic agents increase myocardial contractility, and are used to support cardiac function in conditions such as decompensated congestive heart failure, cardiogenic shock, septic shock, myocardial infarction, cardiomyopathy, etc.

Fall B

En 65-årig diabetiker med KAD samt viss nefropati känd sedan tidigare insjuknar med frossa samt 39- 40°C feber.Vidare noteras grumlig illaluktande urin. Han inkommer med starkt påverkat AT med blåmarmorering perifert och med snabb puls och blodtryck 90/50. Akutprover visar LPK 20 varav 18 poly samt ett serumkreatinin på 180 μmol/L. Patientens vikt är 70 kg.

Handläggning?

Svår sepsis = sviktande organfn (AT, blåmarmorerad) + sepsis (tackykardi, vita, feber)

Misstänkt fokus: njurar, pyelonefrit, bakteriell

  • Enterokocker - inte så här virulent - behöver inte tänka på

Förvarna IVA (narkos), men troligen –> MIMA/MAVA, tät övervakning

Fall C

Som konsult på en kirurgavdelning träffar du en 70-årig dam som 1 vecka tidigare opererats med elektiv colonkirurgi för en coloncancer. Man har genomfört en colonresektion.

Damen är nu högfebril med 40°C feber och har frossat vid flera tillfällen.

Labmässigt noterar du LPK 18.0 med neutrofila 15. Förutom den höga febern tycks allmäntillståndet vara ganska gott och kreatinin normalt.

Handläggning?

Vanligaste nosokomiala infektionerna: lungor, UVI

Buk: anaeroba: E coli, bacterioides, G- stavar –>

  • Piptaz monoterapi räcker (tar även pseudomonas)
  • Alt: cefotaxim + metronidazol (imidazolderivat)
  • Alt: meropenem

Fall D

En 20-årig kvinnlig heroinist söker med hög feber 40°. Inga blåsljud. Sökte för två dagar sedan för samma besvär men avvek. Innan hon avvek togs blododlingar som nu visar staph aureus i 4/4 flaskor. Röntgen visar bollformade infiltrat i båda lungorna. Patienten väger 50 kg och har ett kreatinin på 120 μmol/L.

Handläggning?

Troligen: Högersidig NVE (native valvular endocarditis) - inga blåsljud, lungor, narkoman, s aureus

Bollformade infiltrat = septiska embolier från endokardit (tricuspidalis) - har oftast inte blåsljud

Agens: pneumokocker, S aureus, KNS

ABCDE

Handläggning generellt av endokardit: blododl minst 3 par, förlängd odlingstid, olika lokaler över 1-2h. TEE. Lab (CRP, SR, blodstatus, elstatus), rtg C/P, EKG, riktad rtg

Prover:

  • TTE (transtorakal ekokardiografi) för diagnos av endokardit

AB: kloxacillin (Strep, S aureus; abscesser)

Empirisk beh generellt av NVE: 4-6 veckor m iv ab

  • Om S aureus kan uteslutas: bensyl-Pc 3gx4 + aminoglykosid 3 mg/kgx1
  • Om S aureus ej kan uteslutas: kloxacillin (ekvacillin) 3gx4 + aminoglykosid 5 mg/kgx1
  • Om CNS engagerat: Cefotaxim(Claforan) 3gx(3 - 4) 10 första dagarna

Hö-sidig endokardit (trikuspidalis) helt okomplicerad kan behandlas 2 v 4 v m septisk emboli till lungan - Lisa Abbé:

Strikt högersidig NVE orsakad av S. aureus (MSSA) hos intravenös missbrukare

Okomplicerade fall

inga tecken på septiska nedslag, inget omfattande klaffengagemang, inte stora vegetationer

kloxacillin 12g/dygn (8-16 g beroende på vikt, ålder och njurfunktion) iv fördelat på 3-4 (ev x 6 om allvarlig) doser i 2 v

eventuellt med tillägg av aminoglykosid, ex gentamicin 3-5 mg/kg/dygn x 1 i 2 v

Komplicerade fall

kloxacillin 12g/dygn (8-16 g beroende på vikt, ålder och njurfunktion) iv fördelat på 3-4 (ev x 6 om allvarlig) doser i 4-6 v

ev längre tids behandling om extrakardiella foci

Septiska embolier ses även i kotpelaren (3 mån), mjälte, huden, septisk artrit, CNS (särskilt vä sidan)

Fall E

En 35-årig kvinna som just kommit hem från Thailand söker på akutmottagningen.

Några dagar efter hemkomsten sjuknar hon in med hög feber som stiger under något dygn och lägger sig på 40°C. Ingen diarré. I status opåverkad med 39°C. Puls 60/minut. CRP 50, LPK 4.0 (varav 1.0 stavkärniga granulocyter). Malaquick negativ.

Handläggning?

A

Inte sepsis (HF, AF, vita, feber).

Inte endokardit (KLINIK: feber, blåsljud, septiska embolier - buksmärta (mjälte), andningsbesvär (lungor), TIA/hemipares (CNS) osv, hematuri, B-symtom, hjärtsvikt, hudmanifestationer)

    • G- stav
    • opastöriserad get- o fårost
    • lågintensiva symtom, spondodiskit (?), potent labsmitta: “OBS: misstänkt brucella”
    • lång ab-behandling
  • malaria

    • virus –> serologi, PCR
    • risk: hemorragisk dengue efter några dygn
    • låga TPK, låga leukocyter, CRP ca 50

Behandling

iv cefalosporiner: klaforan

azitromycin

Azithromycin is an antibiotic useful for the treatment of a number of bacterial infections.[1] This includes middle ear infections, strep throat, pneumonia, traveler’s diarrhea, and certain other intestinal infections. It may also be used for a number of sexually transmitted infections including chlamydiaand gonorrhea infections. Along with other medications, it may also be used for malaria. It can be taken by mouth or intravenously with doses once per day.[1]

Common side effects include nausea, vomiting, diarrhea and upset stomach. An allergic reaction or a type of diarrhea caused by Clostridium difficile is possible. No harm has been found with use during pregnancy.[1] Its safety during breastfeeding is unclear but likely okay.[2] Azithromycin is an azalide, a type of macrolide antibiotic. It works by decreasing the production of protein, thus stopping bacterial growth.

Hepatit A - vårdguiden

Symtom

Det finns tre typiska sjukdomsfaser som de flesta vuxna som insjuknar går igenom.

Först känns det som att man har fått influensa med huvudvärk, feber, trötthet och allmän värk i kroppen. I nästa fas kan man tappa aptiten, må illa och ibland kräkas. Till sist brukar huden och ögonvitorna bli gulaktiga. Samtidigt som den gula färgen ökar försvinner de tidiga symtomen, men tröttheten kan finnas kvar länge. Man kan också få ledvärk och klåda.

Hepatit A är en ovanlig sjukdom i Sverige men den är mycket vanlig i länder med sämre hygienisk standard. I många länder i Asien och Afrika är hepatit A att betrakta som en barnsjukdom. I Sverige får varje år omkring 100 personer hepatit A och ungefär hälften har smittats på utlandsresor.

Behandling

Det finns ingen särskild behandling mot hepatit A utan sjukdomen går över av sig själv.

Blodprov: leverstatus och serologi (anti-HAV total - pågående eller utläkt/vaccination, anti-HAV-IgG - utläkt/vaccination, anti-HAV-IgM - akut sjd)

Sepsis

Är systemiska inflammatoriska tillstånd som kan utvecklas från olika infektioner. Vanliga, men ändå en liten andel av sjh-vårdade infektioner. Mortalitet 20 - 30 %.

Patofysiologi

Systemiska följder av aktiveringen av det medfödda immunsvaret:

    • vasodilatation arteriellt och venöst
    • kapillärt läckage
    • mikrotrombotisering
    • vävnadsödem 􏰀

Försämrat syrgasutnyttjande i vävnaderna

+ hypovolemi pga ökad kärlpermeabilitet, vasodilatation och dehydrering och hjärtmuskelpåverkan

+ försämrat syrgasutbyte i lungorna pga ovanstående

􏰀Försämrad leverans av syrgas till vävnaderna –> 􏰀cellulär hypoxi 􏰀–> anaerob metabolism –>􏰀 laktatansamling och metabol acidos

Kliniska symptom och organeffekter:

Temp ↑, eller ibland ↓ (< 36o C) (värre att ha låg än hög temp) Andningsfrekvens ↑ (> 20 varningssignal, 30 allvarligt tecken – OBS AF↑ tidigaste och vanligaste symptomet vid svår sepsis – missas ofta!) Saturation

Blodtryck ↓ Puls ↑

Klinisk bild

Insjuknande i sepsis är som regel plötsligt med frossa, feber och allmänsymtom i form av nytillkommen:

• Andnöd/andfåddhet • Konfusion/motorisk oro • Kräkning och eller diarré • Buksmärta • Allmän svaghet (”dåligt AT”) • ”Blivit funnen på golvet”

Epidemiologi

200/100 000 fall av svår sepsis/år i SE, varav 20 % dör.

Svarar mot 18 000 fall o 3500 döda.

Def av sepsis

SEPSIS = infektion med tecken på kraftig allmän inflammation

Minst 2 av:

  • tackykardi (HF>90)
  • tackypne (AF >20)
  • onormalt antal vita blodkroppar (LPK >12 el. <4)
  • onormal temperatur (temp >38° el. <36°)

SVÅR SEPSIS = SEPSIS med:

  • hypotension (SBT<90) och/eller
  • hypoperfusion (laktat > 1 mmol/l över övre normalgräns) och/eller
  • sviktande organfunktioner

SEPTISK CHOCK= fortsatt hypotension (SBT<90) trots vätskebolus

Inte alltid typiska symtom - fallgropar:

  • Andningsfrekvens skall alltid kontrolleras vid misstänkt sepsis även om saturationen är normal!
  • Feber förekommer inte alltid
  • Diarré och kräkningar misstolkas som gastroenterit
  • Diffusa eller lokala smärttillstånd, t ex buksmärtor misstolkas som kirurgiskt problem
  • Fallskador, ”funnen på golvet” misstolkas som neurologiskt, ortopediskt eller medicinskt problem
  • Konfusion vid sepsis misstolkas ofta som stroke
  • Tid labprover kan vara normala

Viktigaste statusfynd o labprover (mina slutsatser)

  • Blodgas (BE ≤ -5 mmol/L eller p-laktat > 1 mmol över övre ref-värdet –> hypoperfusion –> vätskebolus) - svår sepsis
  • BT - svår sepsis, septisk chock
  • Sviktande organfunktioner - svår sepsis
  • Sepsis: AF, puls, LPK (blodstatus), temp

Blodgas

Blodgas tages:

  • vid ankomst/identifiering – om BE ≤ - 5 mmol/l, eller p-laktat > 1 mmol över övre ref-värdet = hypoperfusion –> 􏰀vätskebolus enl nedan
  • efter vätskebolus, ny blodgas. Om försämrat laktat, tag kontakt IVA/MIMA
  • efter 6 timmar: Om sjunkande laktat, stigande BE = gynnsam utveckling, om oförändrat eller försämrat, tag kontakt IVA

Det finns en stark koppling högt laktat- mortalitet, men klen koppling lågt BT-mortalitet i området 70+ systoliskt. De m BT < 70 har hög mortalitet, men är tämligen få.

  • 65 % av kritiskt sjuka pt m sepsis o BT > 90 har laktat > 2,5 mmol/L
  • Av pt m laktat > 4 har 64 % BT > 90

==> kontrollera laktat frikostigt

Odlingar

Blododling = Två omgångar blododlingar om en aerob + anaerob flaska vardera. Kan tas utan tidsintervall, men från olika insticksställen.

Urinodling – OBS detta får inte fördröja insättandet av antibiotika!

Övriga odlingar styrs av misstänkt fokus: t.ex. luftvägar; sputum, svalg, nasofarynx, sår, abscess, likvor, ledvätska, ascites, pleura.

Överväg viss riktad virusdiagnostik med akutsvar, t.ex. influensa snabbtest/PCR

Överväg pneumokock- och legionella-antigen i urin

Kemlab:

Blodgas, CRP, Hb, LPK, TPK. Vätskebalans, Leverprover, DIC-prover, glukos, evt hjärtinfarktprover

Handlingsschema – septisk chock

  • O2 på mask
  • Hastig lägesbedömning
  • Iv infarter
  • Ge vätska – Ringer eller annan kristalloid vätska 1000 ml/30 min
  • Blodprover inklusive blodgas
  • Gör klinisk bedömning, formulera hypoteser
  • Odla från blod, urin, ev luftvägar, sår, likvor, etc
  • Antibiotika
  • Om cirkulationssvikt trots adekvat substitition, kontakta IVA

Behandla rätt, behandla snabbt!

Med adekvat ab sjunker överlevnaden vid septisk chock med ca 1 %/h efter den första timmen

Dosering

  • Vid svår sepsis och septisk chock kan volymen av den interstitiella vätskan mer än fördubblas och göra att antibiotikakoncentrationerna vid normal dosering blir för låga. Distributionsvolymen för vattenlösliga antibiotika såsom betalaktamer är 2‐3 gånger högre än hos friska.
  • Betalaktamer ges därför i en extrados efter halva dosintervallet mellan första och andra dosen – t ex vid 0,4,8,16 (och 24 timmar) det första dygnet.

Vad är Duke-kriterierna?

B symptoms refer to systemic symptoms of fever, night sweats, and weight loss which can be associated with both Hodgkin’s lymphoma and non-Hodgkin’s lymphoma. The presence or absence of B symptoms has prognostic significance and is reflected in the staging of these lymphomas.

B symptoms are so called because Ann Arbor staging of lymphomas includes both a number (I-IV) and a letter (A or B);.[1] “A” indicates the absence of systemic symptoms, while “B” indicates their presence.

B symptoms include:

  • Fever greater than 38 °C. Pel-Ebstein fever, the classic intermittent fever associated with Hodgkin disease, occurs at variable intervals of days to weeks and lasts for 1–2 weeks before resolving. However, fever associated with lymphoma can follow virtually any pattern.
  • Drenching sweats, especially at night.
  • Unintentional weight loss of >10% of normal body weight over a period of 6 months or less.

Definition

Endokardit innebär infektion av mikroorganism i hjärtklaff eller endokardium. Endokardit definieras enligt Duke-kriterierna:

Major kriterier

  • Minst två positiva blododlingar med ”typisk” mikroorganism eller positiv odling från vegetation.
  • Histologi som visar aktiv endokardit.
  • Ekokardiografi visar vegetation på klaff, stödjevävnad eller implanterat material.
  • Nytillkommet blåsljud.

Minor kriterier

  • Predisponerande hjärtsjukdom eller intravenöst missbruk.
  • Feber, minst 38,0 ºC.
  • Tecken på arteriell emboli, septisk lunginfarkt, mykotiskt aneurysm, konjunktival blödning mm.
  • Immunologiska reaktioner, t ex glomerulonefrit, Oslers knutor, Roth’s spots i ögat, positiv RA-faktor.
  • Endast en positiv blododling (gäller ej S. epidermidis/koagulasnegativa stafylokocker, förutom vid hjärklaffsprotes) eller serologiska tecken på infektion med organism som kan orsaka endokardit.
  • Ej helt typiska fynd på ekokardiografi.

Definitiv endokardit, två av ovanstående major-kriterier eller ett major kriterium + tre minor-kriterier eller vid fem minorkriterier.

Klinisk bild

Man kan skilja på fyra olika sjukdomskategorier beroende på lokalisation.

Endokardit som drabbar egen/nativ klaff på vänster sida. Utgör 80 % av alla endokarditer. Vanligaste agens är alfastreptokocker (30 %), stafylokocker (30 %) och enterokocker (ca 10 %). Mindre vanligt är beta-streptokocker (4 %), samt ”HACEK” (3 %). Dessa senare omfattar Hemophilus species, Actinobacillus, Cardiobakterium, Eikenella och Kinginella. Fynd av något av dessa fem skall föranleda misstanke på endokardit.

Protesendokardit Första året dominerar koagulasnegativa stafylokocker (KNS). Svamp, candida förekommer. Stafylokocker ses både tidigt och sent. Biologisk klaff med sen infektion har samma spektrum som nativ klaffinfektion.

Isolerad högersidig endokardit. Missbrukare. Nästan alltid trikuspidalisklaffen. Vanligen stafylokocker.

Endokardit i anslutning till pacemaker.

Nosokomial endokardit efter sepsis, intensivvård mm utgör 10-15 % av alla endokarditer. Ofta obduktionsfynd. Svampetiologi i högre frekvens.

Symtomen är ospecifika och högst variabla.

  1. Ofta långdragen men måttlig feber, framför allt vid endokardit orsakad av alfastreptokocker (endokarditis lenta). Mer akut insjuknande med septisk feber och allmänpåverkan vid stafylokockendokardit.
  2. Värk i bröstkorgen, leder, muskler. Trötthet.
  3. Hjärtfynd. Nytillkommet blåsljud. Hjärtvikt är ett allvarligt tecken. Vegetationer kan embolisera till hjärtkärlen och ge infarkt. En hjärtabscess kan ge arytmier (AV-block) och ger i regel också ökning av troponin.
  4. Neurologiska symtom pga embolisering ses hos en tredjedel. Embolier kan orsaka mykotiskt aneurysm i CNS, i värsta fall ruptur och subaraknoidalblödning.
  5. Luftvägssymtom vid septisk lunginfarkt.

Särskilt hög risk att insjukna har patienter med tidigare endokardit, mekanisk hjärtklaff eller pågående intravenöst missbruk. Ökad risk även hos äldre, personer med känd klaffsjukdom, medfött hjärtfel eller dialys.

I vissa fall har endokarditen nyligen föregåtts av invasivt ingrepp, operation eller tandrotsbehandling. De flesta fallen har dock inte något samband med tidigare ingrepp. Beträffande antibiotikaprofylax till riskpatienter, se särskilt vårdprogram under kardiologikapitlet.

Utredning

  • Blodprover: Blod-, elektrolytstatus. CRP. SR. Glukos. CRP ofta endast lätt-måttligt förhöjt (20-50 mg/l) vid endokarditis lenta. Troponin.
  • Odlingar: Upprepade blododlingar, minst x 3. Begär förlängd odlingstid 10 dagar. Begär MIC-bestämning. Odla från eventuellt fokus om möjligt. I ca 10 % av diagnosticerade fall är odlingarna negativa.
  • Daglig temperaturmätning och hjärt-/lungauskultation.
  • Följ EKG vid arytmimisstanke. Nytillkommet AV-block kan indikera abscess.
  • Ekokardiografi. Om vegetation inte säkert kan dokumenteras och klinisk misstanke föreligger skall man utföra transesofagal ekokardiografi som påvisar vegetationer hos ca 95 %. Abscesser i myokardiet upptäcks även lättare med denna metod.
  • Lungröntgen med frågeställning hjärtsvikt, septiska embolier (i vissa fall).

Behandling

Obehandlad endokardit är vanligen letal. Alltid inläggningsfall vid misstanke. Kontakta infektionsklinik. Antibiotikaterapin justeras efter odlingssvaret.

Misstänkt eller säkerställd akut endokardit med stark stafylokockmisstanke. Inj Cloxacillin (Ekvacillin) 3 g x 4 iv, vid svåra fall initialt tillägg av inj gentamicin (Garamycin) 3-5 mg/kg x 1 iv under 3-5 dagar. Total behandlingstid i normalfallet 4-6 veckor. Vid strikt högersidig okomplicerad endokardit finns dock evidens för 2 veckors iv behandling.

Misstänkt eller säkerställd subakut endokardit (framför allt alfastreptokocker, enterokocker). Inj Bensylpenicillin 3 g x 4 iv + inj aminoglykosid (t ex Garamycin) 3 mg/kg x 1 iv. Behandlingstid med aminoglykosid vid alfastreptokockendokardit beror på bakteriestammens MIC-värde. Vid bekräftad enterokockendokardit byte till ampicillin + aminoglykosid (se även vårdprogram på infektion.net).

Vid penicillinallergi typ 1 ersätts penicillin med inj vancomycin (Vancocin) 15 mg/kg x 2 iv (doseras efter plasmakoncentration. Mål: dalvärde 15-20 mg/l). Protesendokardit. Empirisk behandling före odlingssvar: Inj vancomycin (Vancocin) 15 mg/kg x 2 iv (doseras efter plasmakoncentration, se ovan) samt inj aminoglykosid (t ex Garamycin) 1,5 mg/kg x 2 iv (koncentrationsbestämning). Vid misstänkt eller bekräftad stafylokock-genes överväger man efter en vecka tillägg av rifampicin (Rifadin) 300-450 mg x 2 po eller iv.

Antikoagulantia och samtidig endokardit

Vid warfarinbehandling och stafylokockinfektion utsätter man warfarin i 1-2 veckor. Under denna tid ges trombosskydd med LMH (inj Klexane 100 mg/ml, 0,4 ml x 2 sc eller inj Fragmin 100 E/kg/dag sc). Vid annan genes än stafylokocker och behandling med warfarin så fortsätter man med antikoagulantia. Patient som står på ASA kan behålla detta.

Protesendokardit med svår klaffdysfuntion och svårbehandlad hjärtsvikt.

Positiv blododling trots 7-10 dagars behandling.

Recidiverande embolier trots ”effektiv” antibiotikabehandling.

Vegetation >10 mm + svår aorta- eller mitralisklaffdysfunktion eller svikt eller fortsatta infektionstecken.

Isolerad stor vegetation, >15 mm.

Prognos

Ca 600 fall av endokardit per år i Sverige. Mortalitet omkring 10 %. Sämst prognos vid hjärtsvikt, hög ålder, engagemang av multipla klaffar, fördröjd behandling, mykotiska aneurysm och stora vege­tationer. Högersidig endokardit har bättre prognos än vänstersidig.

Nationellt vårdprogram

De inflammatoriska systemen har utvecklats för att bekämpa bakterier och främmande agens, som kommit in i kroppen via hud eller slemhinna. I samband med detta uppkommer såväl lokala som systeminflammatoriska symtom. Om de sistnämnda blir för kraftiga finns risk för utveckling mot svår sepsis och septisk chock. I det övervägande antalet fall är det bakteriella infektioner som orsakar dessa allvarliga septiska tillstånd. De vanligaste ingångsportarna för dessa infektioner är nedre luftvägar, urinvägar, mag- tarmkanal, sår och intravaskulära katetrar. Kunskapen om patofysiologin vid septiska tillstånd har ökat avsevärt under senare år men baseras främst på vad man funnit vid gramnegativ infektion.

Endotoxin från gramnegativa och exotoxiner från grampositiva bakterier aktiverar makrofager och ibland andra celler till att producera IL-1 (interleukin-1), TNF (tumor necrosis factor) och andra cytokiner. Dessa, i sin tur, samspelar med varandra, med ytterligare andra cytokiner och med aktiverade kallikrein-kinin-, koagulations-, fibrinolys-, och komplementsystem. Detta leder bland annat till generell aktivering av de neutrofila granulocyterna, som adhererar till endotelet i kärlväggen i många organ med ökad kärlpermeabilitet som följd. Efter initial aktivering av såväl koagulations- som fibrinolyssystem sker i de svårare fallen en hämning av fibrinolyssystemet, vilket medför ökad risk för trombosbildning.

För att motverka skadliga effekter nedregleras den inflammatoriska reaktionen via en mängd hämmare som verkar på olika nivåer i de inflammatoriska systemen. Så kan t ex IL-1 hämmas av en receptorantagonist, IL-1ra, och TNF av lösliga receptorer, vilka fäster till TNF innan bindning till cellbunden receptor sker. Därtill produceras kortison och ett flertal cytokiner och andra mediatorer med antiinflammatorisk effekt. Hos vissa patienter blir emellertid aktiveringen av de proinflammatoriska systemen så kraftig att de hämmande systemen, som aktiveras något senare, inte förmår eller hinner bromsa dessa i tillräcklig grad. Graden av aktivering är inte bara ett resultat av invaderande bakterier utan påverkas också i stor utsträckning av andra faktorer, som genetisk predisposition, läkemedel och olika grundsjukdomar. Av betydelse för inflammationens aktiveringsgrad är också om de antiinflammatoriska mekanismerna redan har aktiverats på grund av en föregående infektion eller ett föregående trauma.

Vid septisk chock leder frisatta substanser som TNF, PAF (platelet activating factor), bradykinin, prostaglandiner och kväveoxid till en vasodilatation på såväl artär- som vensida. Generell vasodilatation, mikrotrombotisering och vävnadsödem leder så småningom till att syrgasutnyttjandet i organen försvåras. Till detta kommer hypovolemi på grund av ökad kärlpermeabilitet, vasodilatation och dehydrering. I de allvarligare fallen leder den inflammatoriska reaktionen till försämrat gasutbyte i lungan och utveckling av ARDS (acute respiratory distress syndrome), vilket i så fall ytterligare bidrar till att försvåra leveransen av syrgas till kroppens organ. Den cellulära hypoxin leder till anaerob metabolism med metabol acidos och laktatansamling som följd. Hypoxi och inflammation samverkar sedan vid utveckling av sviktande organfunktion och MODS (Multiple Organ Dysfunction Syndrome).

I tidigt skede ses ofta en hyperdynamisk cirkulation med hög hjärtminutvolym och lågt systemvaskulärt motstånd. Om hypovolemin inte korrigeras eller patientens hjärta inte orkar med påfrestningarna, minskar hjärtminutvolymen och cirkulationen övergår i en hypodynamisk fas med påslag av det sympatiska nervsystemet. I detta läge är risken stor att tillståndet går över i en irreversibel chock. Av dem som avlider i septisk chock, dör cirka hälften under den första veckan i bilden av en behandlingsrefraktär chock. Övriga dör oftast inom en månad i bilden av multipel organsvikt.

Många av dessa patienter har också tecken på disseminerad intravasal koagulation (DIC) med konsumtion av koagulationsfaktorer och hämmare. Bristen på koagulationsfaktorer kan i de svåraste fallen leda till mer eller mindre spontana blödningar. Dessa ser man oftast först i områden som är eller har varit utsatta för medicinska manipulationer, t ex runt katetrar eller trachealtuber, men de kan också ske var som helst i kroppen.

ARDS

Since World War I, it has been recognized that some patients with nonthoracic injuries, severe pancreatitis, massive transfusion, sepsis, and other conditions develop respiratory distress, diffuse lung infiltrates, and respiratory failure, sometimes after a delay of hours to days.

Before research into the pathogenesis and treatment of this syndrome could proceed, it was necessary to formulate a clear definition of the syndrome. Such a definition was developed in 1994 by the American-European Consensus Conference (AECC) on acute respiratory distress syndrome (ARDS).

ARDS was recognized as the most severe form of acute lung injury (ALI), a form of diffuse alveolar injury. The AECC defined ARDS as an acute condition characterized by bilateral pulmonary infiltrates and severe hypoxemia in the absence of evidence for cardiogenic pulmonary edema. The severity of hypoxemia necessary to make the diagnosis of ARDS was defined by the ratio of the partial pressure of oxygen in the patient’s arterial blood (PaO2) to the fraction of oxygen in the inspired air (FIO2). ARDS was defined by a PaO2/FIO2 ratio of less than 200, and in ALI, less than 300.

This definition was further refined in 2011 by a panel of experts and is termed the Berlin definition of ARDS.[3] ARDS is defined by timing (within 1 wk of clinical insult or onset of respiratory symptoms); radiographic changes (bilateral opacities not fully explained by effusions, consolidation, or atelectasis); origin of edema (not fully explained by cardiac failure or fluid overload); and severity based on the PaO2/FIO2 ratio on 5 cm of continuous positive airway pressure (CPAP). The 3 categories are mild (PaO2/FIO2 200-300), moderate (PaO2/FIO2 100-200), and severe (PaO2/FIO2 ≤100).

Etiology

Multiple risk factors exist for ARDS. Approximately 20% of patients with ARDS have no identified risk factor. ARDS risk factors include direct lung injury (most commonly, aspiration of gastric contents), systemic illnesses, and injuries. The most common risk factor for ARDS is sepsis.

Given the number of adult studies, major risk factors associated with the development of ARDS include the following:

  • Bacteremia
  • Sepsis
  • Trauma, with or without pulmonary contusion
  • Fractures, particularly multiple fractures and long bone fractures
  • Burns
  • Massive transfusion
  • Pneumonia
  • Aspiration

General risk factors for ARDS have not been prospectively studied using the 1994 EACC criteria. However, several factors appear to increase the risk of ARDS after an inciting event, including advanced age, female sex (noted only in trauma cases), cigarette smoking,[4] and alcohol use. For any underlying cause, increasingly severe illness as predicted by a severity scoring system such as the Acute Physiology And Chronic Health Evaluation (APACHE) increases the risk of development of ARDS.

Genetic factors

A study by Glavan et al examined the association between genetic variations in the FAS gene and ALI susceptibility. The study identified associations between four single nucleotide polymorphisms and increased ALI susceptibility.[5] Further studies are needed to examine the role of FAS in ALI.

Prognosis

Until the 1990s, most studies reported a 40-70% mortality rate for ARDS. However, 2 reports in the 1990s, one from a large county hospital in Seattle and one from the United Kingdom, suggested much lower mortality rates, in the range of 30-40%.[8, 9] Possible explanations for the improved survival rates may be better understanding and treatment of sepsis, recent changes in the application of mechanical ventilation, and better overall supportive care of critically ill patients.

Note that most deaths in ARDS patients are attributable to sepsis (a poor prognostic factor) or multiorgan failure rather than to a primary pulmonary cause, although the recent success of mechanical ventilation using smaller tidal volumes may suggest a role of lung injury as a direct cause of death.

MODS

Multiple organ dysfunction syndrome (MODS) is a continuum, with incremental degrees of physiologic derangements in individual organs; it is a process rather than a single event. Alteration in organ function can vary widely from a mild degree of organ dysfunction to completely irreversible organ failure. The degree of organ dysfunction has a major clinical impact.

Eventually, the term MODS was proposed as a more appropriate description. MODS is defined as a clinical syndrome characterized by the development of progressive and potentially reversible physiologic dysfunction in 2 or more organs or organ systems that is induced by a variety of acute insults, including sepsis.

DIC

Disseminated intravascular coagulation (DIC) is characterized by systemic activation of blood coagulation, which results in generation and deposition of fibrin, leading to microvascular thrombi in various organs and contributing to multiple organ dysfunction syndrome (MODS).[1] Consumption and subsequent exhaustion of coagulation proteins and platelets (from ongoing activation of coagulation) may induce severe bleeding, though microclot formation may occur in the absence of severe clotting factor depletion and bleeding.[2]

Derangement of the fibrinolytic system further contributes to intravascular clot formation, but in some cases, accelerated fibrinolysis may cause severe bleeding. Hence, a patient with disseminated intravascular coagulation (DIC) can present with a simultaneously occurring thrombotic and bleeding problem, which obviously complicates the proper treatment.

The subcommittee on DIC of the International Society on Thrombosis and Haemostasis has suggested the following definition for DIC: “An acquired syndrome characterized by the intravascular activation of coagulation with loss of localization arising from different causes. It can originate from and cause damage to the microvasculature, which if sufficiently severe, can produce organ dysfunction.”[3]

DIC is estimated to be present in as many as 1% of hospitalized patients.[4] DIC is not itself a specific illness; rather, it is a complication or an effect of the progression of other illnesses. It is always secondary to an underlying disorder and is associated with a number of clinical conditions, generally involving activation of systemic inflammation.

Clinical continuum of sepsis

Sepsis is a clinical syndrome that complicates severe infection and is characterized by systemic inflammation and widespread tissue injury. A continuum of severity from sepsis to septic shock and MODS exists. The clinical process usually begins with infection, which potentially leads to sepsis and organ dysfunction.[2] A consensus panel of the American College of Chest Physicians and the Society of Critical Care Medicine developed definitions of the various stages of this process: SIRS -> sepsis -> svår sepsis -> septisk chock.

Infection is usually a microbial phenomenon in which an inflammatory response to the presence of microorganisms or the invasion of normally sterile host tissue by these organisms is characteristic. However, viral infections can be indistinguishable from bacteria infections in their presentation.

Bacteremia is the presence of viable bacteria in the blood.

Systemic inflammatory response syndrome (SIRS) may follow a variety of clinical insults, including infection, pancreatitis, ischemia, multiple trauma, tissue injury, hemorrhagic shock, or immune-mediated organ injury. SIRS is a nonspecific presentation of these insults and is defined by the presence of 2 or more of the following:

  • Temperature greater than 38.0°C or less than 36.0°C
  • Heart rate higher than 90 beats/min
  • Respiratory rate higher than 20 breaths/min or arterial carbon dioxide tension below 32 mm Hg
  • White blood cell (WBC) count higher than 12,000/µL, lower than 4000/µL, or including more than 10% bands

Sepsis is a systemic response to infection. I_t is identical to SIRS, except that it must result specifically from infection_ rather than from any of the noninfectious insults that may also cause SIRS. That sepsis and SIRS are phenotypically similar underscores a common inflammatory pathway causing both.

Septic shock is sepsis with hypotension (systolic blood pressure < 90 mm Hg or a reduction of 40 mm Hg from baseline) despite adequate fluid resuscitation. Concomitant (=samtidig) organ dysfunction or perfusion abnormalities (eg, lactic acidosis, oliguria, and coma) are present in the absence of other known causes.

MODS is the presence of altered organ function in an acutely ill patient such that homeostasis cannot be maintained without intervention. Primary MODS is the direct result of a well-defined insult in which organ dysfunction occurs early and can be directly attributable to the insult itself. Secondary MODS develops as a consequence of a host response and is identified within the context of SIRS. The inflammatory response of the body to toxins and other components of microorganisms causes the clinical manifestations of sepsis.

Pathophysiology

Malignant intravascular inflammation

Sepsis has been referred to as a process of malignant intravascular inflammation. Normally, a potent, complex, immunologic cascade ensures a prompt protective response to microorganism invasion in humans. A deficient immunologic defense may allow infection to become established; however, an excessive or poorly regulated response may harm the host through maladaptive release of indigenously generated inflammatory compounds (see the image below).

Lipid A and other bacterial products release cytokines and other immune modulators that mediate the clinical manifestations of sepsis. Interleukins, tumor necrosis factor (TNF)-α, interferon gamma (IFN-γ), and other colony-stimulating factors are produced rapidly within minutes or hours after interactions of monocytes and macrophages with lipid A.

Inflammatory mediator release becomes a self-stimulating process, and release of other such mediators, including interleukin (IL)-1, platelet activating factor, IL-2, IL-6, IL-8, IL-10, and nitric oxide (NO), further increases cytokine levels. This leads to continued activation of polymorphonuclear leukocytes (PMNs), macrophages, and lymphocytes; proinflammatory mediators recruit more of these cells. All of these processes create a state of destructive immunologic dissonance.

Sepsis is described as an autodestructive process that permits extension of the normal pathophysiologic response to infection to involve otherwise normal tissues and results in MODS. Organ dysfunction or organ failure may be the first clinical sign of sepsis, and no organ system is immune from the consequences of the inflammatory excesses of sepsis. Mortality increases as organ failure increases.

Although uncontrolled, once MODS develops systemic evidence of both proinflammatory and anti-inflammatory up-regulation are usually present, suggesting that failure of host defense homeostasis is the final pathway from sepsis to MODS, rather than simple hypotension-induced end-organ injury, as may occur with hemorrhagic shock. Survival from severe sepsis with MODS is usually associated with a generalized reduction in both the proinflammatory and anti-inflammatory response.

A novel hypothesis has recently emerged that survival from severe sepsis requires a generalized down-regulation of the body’s immune response, energetic functions, and associated organ performance. Thus, MODS may by the host’s adaptive response to overwhelming inflammation, allowing inflammation to clear without causing permanent end-organ harm. As discussed below, all organs reveal a generalized hyporesponsiveness that is clearly abnormal in health but may mark a survival strategy in severe sepsis.

Dysfunction of organ systems

Circulatory derangement

Significant derangement in autoregulation of circulation is typical of sepsis. Vasoactive mediators cause vasodilatation and increase microvascular permeability at the site of infection. NO plays a central role in the vasodilatation of septic shock. Also, impaired secretion of vasopressin may occur, which may permit persistence of vasodilatation.

Changes in both systolic and diastolic ventricular performance occur in sepsis. Through the use of the Frank-Starling mechanism, cardiac output often is increased to maintain blood pressure in the presence of systemic vasodilatation. Patients with preexisting cardiac disease are unable to increase their cardiac output appropriately.

Regionally, sepsis interferes with the normal distribution of systemic blood flow to organ systems. Consequently, core organs may not receive appropriate oxygen delivery, and the result is what is known as regional hypoperfusion.

Microcirculation is the key target organ for injury in sepsis since vascular endothelium is universally affected by the circulating inflammatory mediators. Although it is unclear if microcirculatory abnormalities are the cause or an innocent bystander of the end-organ injury, clear microvascular dysfunction is seen. A decrease in the number of perfused capillaries is seen, although with application of vasodilator therapies, full microvascular recruitment occurs. Mitochondrial dysfunction also occurs and is often associated with reduced mitochondrial transmembrane potential gradients, which are necessary to drive oxidative phosphorylation. The end result is an apparent inability of end-organs to extract oxygen maximally.

Debate continues as to whether this failure of energy metabolism is an adaptive cytoprotective mechanism similar to hibernation or reflects primary mitochondrial pathology. These are areas of active research but do not presently translate into clear clinical practice guidelines. Increased capillary endothelial permeability leads to widespread protein-rich tissue edema.

Septic shock and SIRS are characterized by reversible myocardial depression, which can prove resistant to catecholamine and fluid administration. Circulating “myocardial depressant factor”—probably representing the synergistic effects of TNF-α, IL-1β, other cytokines, and NO—is implicated in pathogenesis. The two characteristics of this acute stress myocardial depression are impaired adrenergic responsiveness and diastolic dysfunction leading to relative catecholamine resistance and small rather than dilated hearts. Macrovascular myocardial ischemia and hypoperfusion are unlikely contributors.

In severe sepsis and septic shock, microcirculatory dysfunction and mitochondrial depression cause regional tissue distress, and regional dysoxia therefore persists. This condition is termed microcirculatory and mitochondrial distress syndrome (MMDS).[4] Sepsis-induced inflammatory autoregulatory dysfunction persists, and oxygen need is not matched by supply, leading to MODS.

Redistribution of intravascular fluid volume resulting from reduced arterial vascular tone, diminished venous return from venous dilation, and release of myocardial depressant substances causes hypotension.

Pulmonary dysfunction

Endothelial injury in the pulmonary vasculature leads to disturbed capillary blood flow and enhanced microvascular permeability, resulting in interstitial and alveolar edema. Neutrophil entrapment within the pulmonary microcirculation initiates and amplifies the injury to alveolar capillary membranes. Acute lung injury and acute respiratory distress syndrome (ARDS) are frequent manifestations of these effects. Indeed, sepsis and pneumonia are the most common causes of ARDS.

Gastrointestinal dysfunction

The gastrointestinal (GI) tract may help propagate the injury of sepsis. Overgrowth of bacteria in the upper GI tract may be aspirated into the lungs, producing nosocomial or aspiration pneumonia. The normal barrier function of the gut may be affected, allowing translocation of bacteria, endotoxins, and normal digestive proteases into the systemic circulation and extending the septic response.

Septic shock can cause paralytic ileus that can lead to a delay in the institution of enteral feeding. Excess NO production is thought to be the causative agent of sepsis-induced ileus. The optimal level of nutritional intake is interfered with in the face of high protein and calorie requirements. Narcotics and muscle relaxants can further worsen GI tract motility.

Liver dysfunction

As a consequence of the role the liver plays in host defense, the abnormal synthetic functions caused by liver dysfunction can contribute to both the initiation and progression of sepsis. The reticuloendothelial system of the liver acts as a first line of defense in clearing bacteria and their products; liver dysfunction leads to a spillover of these products into systemic circulation.

Liver failure (“shocked liver”) can be manifested by elevations in liver enzymes and bilirubin, coagulation defects, and failure to excrete toxins such as ammonia, which lead to worsening encephalopathy.

Renal dysfunction

Acute kidney injury (AKI) often accompanies sepsis. Different etiologies for AKI have been reported, and the cause is typically thought to be multifactorial.[5] The mechanism of AKI is complex but likely involves a decrease in effective intravascular volume resulting from systemic hypotension, direct renal vasoconstriction, release of cytokines, and activation of neutrophils by endotoxins and other peptides, which contribute to renal injury. Still, most animal studies show that renal blood flow is increased, not decreased, in sepsis, though associated with impaired tubular function and a lack of significant histologic evidence of tubular injury.

Central nervous system dysfunction

Involvement of the central nervous system (CNS) in sepsis produces encephalopathy and peripheral neuropathy. The pathogenesis is poorly defined but is probably related to systemic hypotension, which can lead to brain hypoperfusion.

Coagulopathy

Subclinical coagulopathy, signaled by a mild elevation of the thrombin time (TT) or activated partial thromboplastin time (aPTT) or a moderate reduction in the platelet count, is extremely common; however, overt disseminated intravascular coagulation (DIC) may also develop. Protease-activated receptors (PARs), especially PAR 1, form t_he molecular link between coagulation and inflammation_; PAR1 exerts cytoprotective effects when stimulated by activated protein C or low-dose thrombin but exerts disruptive effects on endothelial-cell barrier function when activated by high-dose thrombin.[6]

Mechanisms of organ dysfunction and injury

The precise mechanisms of cell injury and resulting organ dysfunction in sepsis are not fully understood. MODS is associated with widespread endothelial and parenchymal cell injury, some of which can be explained by the following 4 proposed mechanisms.

Hypoxic hypoxia

The septic circulatory lesion disrupts tissue oxygenation, alters the metabolic regulation of tissue oxygen delivery, and contributes to organ dysfunction. Microvascular and endothelial abnormalities contribute to the septic microcirculatory defect in sepsis. The reactive oxygen species, lytic enzymes, and vasoactive substances (eg, NO and endothelial growth factors) lead to microcirculatory injury, which is compounded by the inability of the erythrocytes to navigate the septic microcirculation.

Direct cytotoxicity

Endotoxin, TNF-α, and NO may cause damage to mitochondrial electron transport, leading to disordered energy metabolism. This is called cytopathic or histotoxic anoxia, an inability to utilize oxygen even when it is present.

Apoptosis

Apoptosis (programmed cell death) is the principal mechanism by which dysfunctional cells are normally eliminated. The proinflammatory cytokines may delay apoptosis in activated macrophages and neutrophils, but other tissues (eg, gut epithelium), may undergo accelerated apoptosis. Therefore, derangement of apoptosis plays a critical role in the tissue injury of sepsis.

Immunosuppression

The interaction between proinflammatory and anti-inflammatory mediators may lead to an imbalance between them. An inflammatory reaction or an immunodeficiency may predominate, or both may be present.

Host response and other factors influencing outcome

Clinical characteristics that relate to the severity of sepsis include the host response to infection, the site and type of infection, the timing and type of antimicrobial therapy, the offending organism, the development of shock, the underlying disease, the patient’s long-term health condition, and the number of failed organs. Factors that lead to sepsis and septic shock may not be essential in determining the ultimate outcome.

The host response to sepsis is characterized by both proinflammatory responses and anti-inflammatory immunosuppressive responses. The direction, extent, and duration of these reactions are determined by both host factors (eg, genetic characteristics, age, coexisting illnesses, medications) and pathogen factors (eg, microbial load, virulence).[7]

Inflammatory responses are initiated by interaction between pathogen-associated molecular patterns expressed by pathogens and pattern recognition receptors expressed by host cells at the cell surface (toll-like receptors [TLRs] and C-type lectin receptors [CLRs]), in the endosome (TLRs), or in the cytoplasm (retinoic acid inducible gene 1–like receptors [RLRs] and nucleotide-binding oligomerization domain–like receptors [NLRs]).[7]

The consequence of exaggerated inflammation is collateral tissue damage and necrotic cell death, which results in the release of damage-associated molecular patterns, so-called danger molecules that perpetuate inflammation at least in part by acting on the same pattern-recognition receptors triggered by pathogens.

  • Severe hepatic failure
  • Severe toxic reactions - Envenomations, transfusion reactions, and transplant rejection
  • Heat stroke and hyperthermia
  • Hemorrhagic skin necrosis (purpura fulminans) [6, 7]
  • Catastrophic antiphospholipid syndrome (rare) [8]

DIC is most commonly observed in severe sepsis and septic shock. Indeed, the development and severity of DIC correlate with mortality in severe sepsis.[2, 9] Although bacteremia, including both gram-positive and gram-negative organisms, is most commonly associated with DIC, other organisms (eg, viruses, fungi, and parasites) may also cause DIC.

Trauma, especially neurotrauma, is also frequently associated with DIC. DIC is more frequently observed in trauma patients with the systemic inflammatory response syndrome (SIRS).[10] Evidence indicates that inflammatory cytokines play a central role in DIC in both trauma patients and septic patients. In fact, the systemic cytokine profiles in septic patients and trauma patients are nearly identical.[11]

The symptoms of DIC are often those of the underlying inciting condition (see Etiology). In addition, symptoms of thrombosis, embolism, organ dysfunction, or bleeding may be present.

Acute versus chronic DIC

DIC exists in both acute and chronic forms. Acute DIC develops when sudden exposure of blood to procoagulants (eg, tissue factor [TF], or tissue thromboplastin) generates intravascular coagulation. Compensatory hemostatic mechanisms are quickly overwhelmed, and, as a consequence, a severe consumptive coagulopathy leading to hemorrhage develops. Abnormalities of blood coagulation parameters are readily identified, and organ failure frequently results.

In contrast, chronic DIC reflects a compensated state that develops when blood is continuously or intermittently exposed to small amounts of TF. Compensatory mechanisms in the liver and bone marrow are not overwhelmed, and there may be little obvious clinical or laboratory indication of the presence of DIC. Chronic DIC is more frequently observed in patients with solid tumors and in those with large aortic aneurysms.[12]

Pathophysiology

The hematologic derangements seen in DIC result from the following 4 simultaneously occurring mechanisms[13] :

  • TF-mediated thrombin generation
  • Dysfunctional physiologic anticoagulant mechanisms (eg, depression of antithrombin and protein C system), which cannot adequately balance this thrombin generation
  • Impaired fibrin removal due to depression of the fibrinolytic system – This is mainly caused by high circulating levels of plasminogen activator inhibitor type 1 (PAI-1); however, in exceptional forms of DIC, fibrinolytic activity may be increased and contribute to bleeding
  • Inflammatory activation

Thrombin generation and tissue factor

Thrombin generation is detectable at 3-5 hours after the occurrence of bacteremia or endotoxemia. Ample evidence exists for a pivotal role of the TF/factor VIIa system in the initiation of thrombin generation.[14, 15, 16]

Exposure to TF in the circulation occurs via endothelial disruption, tissue damage, or inflammatory or tumor cell expression of procoagulant molecules (including TF). TF activates coagulation via the extrinsic pathway involving factor VIIa. The TF-VIIa complex activates thrombin, which cleaves fibrinogen to fibrin while simultaneously causing platelet aggregation. The intrinsic (or contact) pathway may also be activated in DIC, though contributing more to hemodynamic instability and hypotension than to activation of clotting.[17]

Thrombin produced by the TF/factor VII pathway amplifies both clotting and inflammation through the following activities[18] :

  • Platelet activation, enhancing aggregation and augmenting platelet functions in coagulation
  • Activation of factors VIII, V, and XI, yielding further thrombin generation
  • Enhanced activation of proinflammatory factors via protease-activated receptors (PARs)
  • Activation of factor XIII to factor XIIIa, which augments production of fibrin clots from fibrinogen
  • Activation of thrombin-activatable fibrinolysis inhibitor (TAFI), making clots resistant to fibrinolysis; and
  • Enhanced expression of adhesion molecules (eg, L-selectin), thereby promoting the inflammatory effects of white blood cells (WBCs)

Factor VIIa has been implicated as the central mediator of intravascular coagulation in sepsis. Blocking the factor VIIa pathway in sepsis has been shown to prevent the development of DIC, whereas interrupting alternative pathways did not demonstrate any effect on clotting.[19, 20]

Abrogation of the TF/factor VII(a) pathway by monoclonal antibodies specifically directed against TF or factor VIIa activity completely inhibited of thrombin generation in endotoxin-challenged chimpanzees and prevented the occurrence of DIC and mortality in baboons that were infused with Escherichia coli. Indeed, in most DIC patients, TF antigen is detectable in plasma. Hence, activation of coagulation in DIC is primarily TF-driven; the intrinsic pathway of coagulation appears not to play an important role.

The actual source of the TF has not been established with certainty. TF may be expressed on mononuclear cells in vitro, and its expression on circulating monocytes of patients with severe infection has been demonstrated. In addition, it may be expressed on endothelial cells. Injured endothelial cells express TF, whereas healthy ones do not. However, the importance of endothelial cell TF expression in vivo and its role in the pathogenesis of DIC remain to be determined.

Another source of TF may be polymorphonuclear leukocytes (PMNs) and other similar cell types, though it is unlikely that these cells actually synthesize TF in substantial quantities. On the basis of the observation that TF has been transferred from leukocytes to activated platelets on a collagen surface in an ex-vivo perfusion system, it is hypothesized that this “bloodborne” TF is transferred between cells through microparticles derived from activated mononuclear cells.

Impaired coagulation inhibitor systems

Thrombin generation is usually tightly regulated by multiple hemostatic mechanisms. However, once intravascular coagulation commences, compensatory mechanisms are overwhelmed or incapacitated. Impaired functioning of various natural regulating pathways of coagulation activation may amplify further thrombin generation and contribute to fibrin formation.

Plasma levels of the most important inhibitor of thrombin, antithrombin, are usually markedly reduced in patients with DIC. This reduction is caused by the following:

  • Antithrombin is continuously consumed by ongoing activation of coagulation
  • Elastase produced by activated neutrophils degrades antithrombin as well as other proteins
  • Further antithrombin is lost to capillary leakage
  • Production of antithrombin is impaired secondary to liver damage resulting from underperfusion and microvascular coagulation [12, 21]

Low antithrombin levels in DIC are associated with increased mortality, particularly in patients with sepsis.[16] That low levels of antithrombin precede the clinical manifestation of sepsis in prospective studies suggests that antithrombin is indeed involved in the pathogenesis of this disease and associated organ dysfunction.[22]

In addition to the decrease in antithrombin, significant depression of the protein C system may occur. Protein C, along with protein S, also serves as a major anticoagulant compensatory mechanism. Under normal conditions, protein C is activated by thrombin when complexed on the endothelial cell surface with thrombomodulin.[12] Activated protein C combats coagulation via proteolytic cleavage of factors Va and VIIIa and proteolyzes PAR1 when bound to the endothelial cell protein C receptor (EPCR).

Impaired functioning of the protein C pathway is mainly due to down-regulation of thrombomodulin expression or its inactivation by cellular reactive oxygen species on endothelial cells by proinflammatory cytokines such as tumor necrosis factor-alpha (TNF-α) and interleukin (IL)–1b.[23] This downregulation has been confirmed in studies in patients with meningococcal sepsis.

In combination with low levels of zymogen protein C (due to mechanisms similar to those described for antithrombin), this process results in diminished protein C activation, which will enhance the procoagulant state. Protein C levels are further reduced via consumption, extravascular leakage, renal loss, and reduced hepatic production and by a reduction in circulating free protein S. The availability of protein S to serve as a cofactor for activated protein C is regulated by the degree it is bound to the complement protein C4B-binding protein.

Animal experiments involving severe inflammation-induced coagulation activation convincingly show that compromising the protein C system results in increased morbidity and mortality, whereas restoring adequate functioning of activated protein C improves survival and organ failure. Experiments show that mice with a 1-allele targeted deletion of the protein C gene (resulting in heterozygous protein C deficiency) have more severe DIC and organ dysfunction and a higher mortality than wild-type littermates.

Besides being implicated in the physiologic regulation of thrombin formation, activated protein C probably also has important inflammation-modulating effects, which may be of relevance in the pathogenesis of DIC.

TF pathway inhibitor (TFPI) is another anticoagulant mechanism that is disabled in DIC. TFPI reversibly inhibits factor Xa and thrombin (indirectly) and has the ability to inhibit the TF-VIIa complex. Although levels of TFPI are normal in patients with sepsis, a relative insufficiency in DIC is evident. TFPI depletion in animal models predisposes to DIC, and TFPI blocks the procoagulant effect of endotoxin in humans.[24] The role of TFPI in the pathogenesis of DIC has not been fully clarified.

The experimental finding that administration of recombinant TFPI blocks inflammation-induced thrombin generation in humans, along with the observation that pharmacologic doses of TFPI are capable of preventing mortality during systemic infection and inflammation, suggests that high concentrations of TFPI are capable of modulating TF-mediated coagulation. Presumably, however, the endogenous concentration of TFPI is insufficiently capable of regulating coagulation activation and the downstream consequences during systemic inflammation.

Defective fibrinolysis

The intravascular fibrin produced by thrombin is normally eliminated via a process termed fibrinolysis. Experimental models indicate that at the time of maximal activation of coagulation, the fibrinolytic system is largely shut off.

Experimental bacteremia and endotoxemia result in a rapid increase in fibrinolytic activity, most probably caused by release of plasminogen activators from endothelial cells. However, this profibrinolytic response is almost immediately followed by suppression of fibrinolytic activity due to a sustained increase in plasma levels of PAI-1[25] ; these effects are mediated by TNF-2 and IL-1.[26] High PAI-1 levels precede DIC and predict poor outcomes.[27]

Notably, strategies that can block endotoxin-induced thrombin generation completely, such as anti-TF antibodies or recombinant hirudin (r-hirudin), have no significant effect on activation and subsequent inhibition of fibrinolysis, which suggests that these 2 processes are independently regulated.

In a study of 69 DIC patients (31 with multiorgan failure), higher levels of tissue plasminogen activator (t-PA) antigen and PAI-1 with depressed levels of α2 -antiplasmin were observed in patients with DIC and multiorgan failure than in DIC patients without multiorgan failure.[28] This finding supports the conclusion that fibrinolysis is a mechanism vital to the prevention of multiorgan failure.

Playing a key role in the process of coagulation and hemostasis is the vascular endothelium, which is responsible for the production of von Willebrand factor (vWF). vWF mediates the adhesion between the platelet surface receptors and the vessel wall and is increased in cases of thrombotic microangiopathy related to DIC. Impaired control of endothelial cell thrombomodulin expression may result in facilitated thrombin generation, which subsequently results in increased platelet activation and the conversion of fibrinogen to fibrin.[29]

Rare cases of DIC are characterized by a severe hyperfibrinolytic state on top of an activated coagulation system. Examples of such situations are the DIC that occurs as a complication of acute myeloid leukemia M-3 (in the French-American-British [FAB] classification) or the DIC that may occur secondary to some forms of adenocarcinoma. Although hyperfibrinolysis predominates in this situation, disseminated thrombosis is still found in a considerable number of patients at autopsy. Clinically, however, these patients suffer from severe bleeding.

In general, patients with DIC should not be treated with antifibrinolytic agents, because this may increase the fibrinolytic deficit and may result in increased thrombosis.

Inflammatory activation

Inflammatory and coagulation pathways interact in substantial ways. It is clear that there is cross-communication between the 2 systems, whereby inflammation gives rise to activation of the clotting cascade and the resultant coagulation stimulates more vigorous inflammatory activity. There are a number of different triggers that can cause a hemostatic imbalance, giving rise to a hypercoagulable state.

Many of the activated coagulation factors produced in DIC contribute to the propagation of inflammation by stimulating endothelial cell release of proinflammatory cytokines.[30] Factor Xa, thrombin, and the TF-VIIa complex have each been demonstrated to elicit proinflammatory action. Furthermore, given the anti-inflammatory action of activated protein C and antithrombin, depression of these anticoagulants in DIC contributes to further dysregulation of inflammation.[5, 31, 32]

In 1992, the American College of Chest Physicians (ACCP) and the Society of Critical Care Medicine (SCCM) introduced definitions for systemic inflammatory response syndrome (SIRS), sepsis, severe sepsis, septic shock, and multiple organ dysfunction syndrome (MODS).[1] The idea behind defining SIRS was to define a clinical response to a nonspecific insult of either infectious or noninfectious origin.

SIRS is defined as 2 or more of the following variables (see Presentation and Workup):

  • Fever of more than 38°C (100.4°F) or less than 36°C (96.8°F)
  • Heart rate of more than 90 beats per minute
  • Respiratory rate of more than 20 breaths per minute or arterial carbon dioxide tension (PaCO 2) of less than 32 mm Hg
  • Abnormal white blood cell count (>12,000/µL or < 4,000/µL or >10% immature [band] forms)

SIRS is nonspecific and can be caused by ischemia, inflammation, trauma, infection, or several insults combined. Thus, SIRS is not always related to infection.

Dosering

  • Vid svår sepsis och septisk chock kan volymen av den interstitiella vätskan mer än fördubblas och göra att antibiotikakoncentrationerna vid normal dosering blir för låga. Distributionsvolymen för vattenlösliga antibiotika såsom betalaktamer är 2‐3 gånger högre än hos friska.
  • Betalaktamer ges därför i en extrados efter halva dosintervallet mellan första och andra dosen – t ex vid 0, 4, 8, 16 (och 24 timmar) det första dygnet.

B symptoms refer to systemic symptoms of fever, night sweats, and weight loss which can be associated with both Hodgkin’s lymphoma and non-Hodgkin’s lymphoma. The presence or absence of B symptoms has prognostic significance and is reflected in the staging of these lymphomas.

Description and nomenclature

B symptoms are so called because Ann Arbor staging of lymphomas includes both a number (I-IV) and a letter (A or B);.[1] “A” indicates the absence of systemic symptoms, while “B” indicates their presence.

B symptoms include:

  • Fever greater than 38 °C. Pel-Ebstein fever, the classic intermittent fever associated with Hodgkin disease, occurs at variable intervals of days to weeks and lasts for 1–2 weeks before resolving. However, fever associated with lymphoma can follow virtually any pattern.
  • Drenching sweats, especially at night.
  • Unintentional weight loss of >10% of normal body weight over a period of 6 months or less.

Prognostic importance

The presence of B symptoms is a marker for more advanced disease with systemic, rather than merely local, involvement. B symptoms are a clear negative prognostic factor in Hodgkin lymphoma.[2] The relevance of B symptoms in non-Hodgkin lymphoma is less clear, although here also B symptoms tend to correlate with disease that is either more widespread or of a higher histologic grade.[3]

Relative importance of specific B symptoms

It has been suggested that, in Hodgkin lymphoma, fever and weight loss are much more prognostically significant than night sweats. In one series of patients with early-stage Hodgkin disease, the presence or absence of night sweats had no impact on cure rates and outcome. However, fever and weight loss had a pronounced negative impact on cure and survival rates, regardless of treatment modality.

“B symptoms” in other diseases

Similar systemic symptoms can be found in non-cancerous states such as tuberculosis and various inflammatory or rheumatologic conditions. In these settings, the term “B symptoms” is sometimes colloquially applied to refer to such systemic or constitutional symptoms. However, in a pure sense, the term “B symptoms” is restricted to lymphoma staging.

  • Middle East respiratory syndrome (MERS) is a viral respiratory disease caused by a novel coronavirus (MERS‐CoV) that was first identified in Saudi Arabia in 2012.
  • Coronaviruses are a large family of viruses that can cause diseases ranging from the common cold to Severe Acute Respiratory Syndrome (SARS).
  • Typical MERS symptoms include fever, cough and shortness of breath. Pneumonia is common, but not always present. Gastrointestinal symptoms, including diarrhoea, have also been reported.
  • Approximately 36% of reported patients with MERS have died.
  • Although the majority of human cases of MERS have been attributed to human-to-human infections, camels are likely to be a major reservoir host for MERS-CoV and an animal source of MERS infection in humans. However, the exact role of camels in transmission of the virus and the exact route(s) of transmission are unknown.
  • The virus does not seem to pass easily from person to person unless there is close contact, such as occurs when providing unprotected care to a patient.

Symptoms

The clinical spectrum of MERS-CoV infection ranges from no symptoms (asymptomatic) or mild respiratory symptoms to severe acute respiratory disease and death. A typical presentation of MERS-CoV disease is fever, cough and shortness of breath. Pneumonia is a common finding, but not always present. Gastrointestinal symptoms, including diarrhoea, have also been reported. Severe illness can cause respiratory failure that requires mechanical ventilation and support in an intensive care unit. Approximately 36% of reported patients with MERS-CoV have died. The virus appears to cause more severe disease in older people, people with weakened immune systems, and those with chronic diseases such as cancer, chronic lung disease and diabetes.

Source of the virus

MERS-CoV is a zoonotic virus that is transmitted from animals to humans. The origins of the virus are not fully understood but, according to the analysis of different virus genomes, it is believed that it originated in bats and was transmitted to camels sometime in the distant past.
