what is the post partum period
period from delivery of placenta to 6 weeks after
structure of lactating breast
secretory lobules, emptying into ductules. 15-20 lobules form a duct, which widens into an ampulla. the lactiferous duct carries the secretion to the outside
4 causes of post partum haemorrhage
tone, tissue, thrombin, trauma
primary haemorrhage
up to 24 hours
secondary haemorrhage
24h-6 weeks
causes of maternal collapse/cardiac arrest
amniotic fluid embolism, hypoglycaemia, hypothension
risk factors thromboembolic disease
surgery, obesity, recent birth, immobility.
when do baby blues peak

promote proliferation of alveolar and duct cells
lactogenic hormones
promote initiation of milk production
galactokinetic hormones
promote contraction of myoepithelial cells
galactopoietic hormones
maintain milk production
how does suckling cause PRL release
inhibits neurones from spinal cord to inhibit dopamine release, and levels of PRL therefore release. leads to stimulation of milk production
what normally inhibits PRL release
dopamine secreting neurones in hypothalamus
role of oxytocin
released from neurones in spinal cord, causes myoepithelial cells to contract and eject milk- let down mechanism.
how does breast feeding cause amenorrhea
inhibits arcuate and prepoptic neurones, causing GnRH to fall. this inhibits ovarian cycle
what is breast milk initially
colostrum, high fat and immunoglobulins.
how much milk is produced daily
800ml
how does breast milk prevent infection
lactoferrin, non pathogenic flora for gut, bactericidal enzymes, immunoglobulins, T cells and granulocytes for cell mediated immunity
puerperal mastitis
accumulation fo milk in breast causing inflammation, caused by not breastfeeding with most breasts.
most common organism for breast infection/puerperal mastitis
S aureus.
treatment breast infection/puerperal mastitis
continue feeding and increase frequency. antibiotics after 24 hours
how to stop lactation
milk accumulates in alveoli of breast, causing distension and atrophy of glandular epithelium. mechanical not hormonal
what drugs supress lactation
dopamine agonists eg. bromocriptine
