Symptoms of subacromial impingement syndrome?
- Anterior/Lateral Shoulder Pain
- Primary complaint is pain with overhead activity
- Gradual or sudden onset
- Night pain
Does SIS have a specific cause?
No. May be:
- Tendinopathy
- Bursitis
- Partial Tears
- Complete Tears
What are the risk factors for SIS?
-Smoking, hooked acromion, desk jobs, prone sleeping
What are some extrinsic causes of SIS?
- Acromial shape
- Occupation/sport
- Degenerative joing
- Instability
What are some intrinsic causes of SIS?
- Limited blood flow
- Microtrauma theory
What are prognostic indicators of SIS?
-Poor prognosis from high pain and longer duration
List the neer classifications for rotator cuff tear
- 1: Under age 25, reversible, swelling, tendonitis, no tear, conservative treatment
- 2: Age 25-40, permanent scarring, tendonitis, no tears, subacromial depression
- 3: Over age 40, small RC tear, SAD with debridement and repair
- 4: Over age 40, large tear, SAD with repair
What’s the difference between partial and full thickness tears
-Partial can heal,. full can’t heal
What is the difference between large and small tears
Large >1cm, small <1cm
Can asymptomatic tears become symptomatic?
Yes, 50% do in 3 yars.
What is the most common cause of traumatic tears?
FOOSH
In order, what RC muscles are most commonly torn
-Supraspinatus, subsccapularis, infraspinatus
Is conservative care indicated in traumatic tears?
Less effective, early surgery for large tears and young patients.
What are the risk factors for a degenerative RC tear?
Smoking, hypercholerterolemia, genetcics, age
What are the signs of poor prognosis in RC tears?
Greater pain and less motion, large full thickness,
What are factors that may cause nondislocating shoulder instability?
- Genetics
- Repeated overhead activities
- Stroke
Risk factors for dislocations?
-Male, contact sports, military
What three special tests are signs of instability?
+ sulcus sign, + apprehension test, + jobe relocation test
What are suggested treatments for shoulder disorders in general?
- Mobility (GIRD)
- Motor control of the scapula
- Strengthening/stability
- Pain modulation
- Graded tissue stress
- Education
