A

Prokinetics & Diet

Why use a prokinetic for prevention?

A

(+) MMC to prevent recurrence – Sweep bacteria down into LI out of SI
• High relapse rates without prokinetic

When is a prokinetic started?

A

Soon after finishing Tx

1) Day after finish Tx to prevent rapid relapse (hard to assess Tx effect when starting new rx)
2) Wait a bit to assess Tx result on it’s own, then start Pk- Few days (2-5) or 1-2 weeks (may relapse)

Why use diet for SIBO?

A

Prevention (along with prokinetic): essential
Manage symptoms while treatment is in progress
Decrease systemic sx & improve overall health

How do you find the perfect diet for a patient with SIBO?

A

Trial and error?

Trends: – SCD better for IBD – LFD better for IBS – SSFG better for SIBO

A

Grains, Starchy Veggies, Starch itself, some Beans, Lactose, gums/thickeners, Sucrose and sweeteners (besides Honey and Saccharine)
Also, processed foods: polysacc/disacc contamination
