What does the SHIVERS mnemonic stand for in serotonin syndrome?
• S: Shivering
• H: Hyperreflexia
• I: Increased temperature
• V: Vital sign instability
• E: Encephalopathy (confusion)
• R: Restlessness
• S: Sweating
What causes serotonin syndrome?
Excess serotonin from combining serotonergic meds (e.g., SSRIs + MAOIs, SNRIs, TCAs, triptans, etc.)
What is the treatment for serotonin syndrome?
Discontinue offending agents, supportive care, possibly cyproheptadine
What does the FINISH mnemonic stand for in SSRI/SNRI discontinuation syndrome?
• F: Flu-like symptoms
• I: Insomnia
• N: Nausea
• I: Imbalance (dizziness, ataxia)
• S: Sensory disturbances (brain zaps)
• H: Hyperarousal (anxiety, agitation)
Which SSRIs/SNRIs are most associated with discontinuation syndrome?
Paroxetine, fluvoxamine, venlafaxine (short half-lives)
What causes neuroleptic malignant syndrome (NMS)?
Dopamine blockade from antipsychotics
What are hallmark signs of NMS?
Lead-pipe rigidity, hyperthermia, confusion, elevated CK, autonomic instability
What is the treatment for NMS?
Discontinue antipsychotic, supportive care, bromocriptine or dantrolene
How do you differentiate NMS from serotonin syndrome?
NMS = rigid muscles + slow reflexes
Serotonin syndrome = clonus + hyperreflexia
What are the 4 major types of EPS?
- Acute dystonia (spasms)
- Akathisia (restlessness)
- Parkinsonism (bradykinesia, tremor)
- Tardive dyskinesia (late, involuntary movements)
What causes EPS?
Dopamine D2 blockade in the nigrostriatal pathway
How is acute dystonia treated?
IM or IV benztropine or diphenhydramine
What is the treatment for akathisia?
Propranolol or benzodiazepines
What medication is used to treat tardive dyskinesia?
Valbenazine (Ingrezza) or deutetrabenazine (Austedo)
What’s the BBW for all antidepressants in youth?
Increased risk of suicidal thoughts and behaviors
What’s the BBW for clozapine?
Agranulocytosis, seizures, myocarditis
What’s the BBW for lamotrigine?
Stevens-Johnson syndrome (esp. if titrated too fast)
