A
  1. I am going to be applying a knee-high elastic stocking on you today.
  2. Provides Privacy
  3. Make sure the bed is raised, locked and secured on a supine position
  4. Turn stocking inside-out To the heal and smooth out all around
  5. uncovering the left/right leg.
  6. Stretch and place stocking over the foot, heel & leg gently
  7. “ The stocking is wrinkle free ” Cover resident
  8. Open privacy, Bed is low & locked, signal light, Wash hands.

Assist to Ambulate Using Transfer Belt

A
  1. I will be ambulating you using a transfer belt today.
  2. Provide privacy/ Grab non-skid footwear/ transfer belt
  3. make sure the bed is locked and lowerred. Put on non-skid shoes on resident
  4. Now I will assist you to place you on a sitting position. FEET FLAT ON THE FLOOR
  5. Now before standing, I will apply transfer belt securely at the waist over clothing/gown
  6. [ Before assisting to stand, provide pre-ranged signals ] “ Now I will stabilize your legs, we will stand toe to toe & knee to knee ”
  7. We will be standing on the count of 3

( hold transfer belt cuffed upward position on the back & side)

  1. Walk slightly behind & to the side for a distance of 10ft
  2. Assists resident to bed & removes belt
  3. Privacy, Call light, bed is low & locked, wash hands

Assist with use of bedpan

A
  1. I am here to assist you with the bedpan.
  2. Privacy / Safety check / wash hands/ get supplies
  3. Lowers head of bed/ DON gloves
  4. Places bedpan (wide side towards the headboard)
  5. DOFF gloves into waste container/ wash hands.
  6. Rises head/toilet tissue and hand wipes are within reach.
  7. Instruct resident to clean hands with hand wipes when finished.
  8. Signal light within reach/ ask resident to signal when finished/ bed is low.

_______Step out & Step back in________

  1. lowered bed/ DON gloves before removing the bedpan.
  2. Bend knees & raise hips/pan is removed.
  3. Resident is covered, except when placing and removing bedpan.
  4. Empty, rinse & empty into toilet/places bedpan in designated dirty supply area.
  5. Doffing gloves into Waste container/wash hands.
  6. Unprovide privacy, bed is low and locked, signal lights.

  2. Bottom of sink is lined and/or sink is partially filled with water before denture is held over sink.
  3. Rinse denture in moderate temperature running water

Provides Foot Care On One Foot

  3. Fill Basin & check temperature & ask resident to verify comfort
  4. Uncover foot, place towel, place basin on towel
  5. Don gloves
  6. Residents foot is placed in water
  7. Applies soap to WET washcloth
  8. Lift foot while supporting ankle joint, washes foot including toes.
  9. Grab new washcloth & rinse foot including between the toes
  10. Remove basin to the side, use barrier to dry foot & in between toes.
  11. Applies lotion, NEVER between toes, removes excess with towel
  12. Cover foot, empties, Rinse, dry basin
  13. Places basin in designated dirty supply area
  14. Disposed of used linen into soiled container

15.Doff gloves, privacy, safety check, wash hands

Positioning On Side

  1. Hello is it ok to position you on you’re side?
  2. Privacy (grab supplies)
  3. Make sure the HEAD OF BED is lowered, bed is locked, lift side rail
  4. Assist resident to slowly roll to side towards the side rail (one arm on shoulder blade, one on raised knee)
  5. Adjust pillow under head
  6. Reposition resident to make sure resident is not laying in arm/place pillow
  7. Fold pillow in half & tuck it underneath resident/ ask to lean back
  8. Place pillow between legs with knee flexed, knee & ankle supported, then cover resident
  9. Signal light, safety check, privacy & wash hands

Performs Modified Passive Range of Motion for one knee and one ankle

  1. Today I am going to be exercising your leg
  2. Privacy, bed is locked & on supine position, instruct resident to alert you whenever experiencing pain during exercise
  3. (Uncover one leg) While supporting underneath the ankle & knee joint/ flexion & extension of the knee 3 times
  4. While supporting the Ankle NOT THE HEEL, lift a little and so plantar & dorsiflex 3 times ( verbalize if in pain)
  5. Recover the residents leg
  6. Signal light, safety check, privacy, wash hands

  6. Expose perineal area (hips to knee)
  7. Applies soap to wet washcloth.
  8. Washes genital area, front to back while using clean area of washcloth for each stroke.
  9. Using clean, wash cloth, rinses soap from genital area.
  10. Use barrier iF towel/or grab clean towel to dry.
  11. Turn resident to side, wash rectal area.
  12. Use, clean, washcloth, rinse soap from rectal area
  13. Dries rectal area.
  14. Reposition resident
  15. Empties, rinse and dry’s basin.
  16. Disposes of linen/ Barrier
  17. Doff gloves/washes hands/safety check/privacy.

Transfers From Bed to Wheelchair Using Transfer Belt

  1. Hello, I will be transferring you from the bed to the wheelchair.
  2. Privacy, safety check, grab supplies, [transfer belt, slippers, wheelchair]
  3. Positions wheelchair by head of bed facing foot or foot a bed facing head.
  4. Foot rest of wheelchair are folded up or removed
  5. Lock Wheels On Wheelchair
  6. Safety check bed.
  7. Before assisting to stand, resident is assisted to sitting position with feet flat on the floor.
  8. Client is wearing nonslip shoes.
  9. Applies transfer belt securely at the waist over gown
  10. Before assisting to stand, provide instructions to begin standing.
  11. Stan, facing resident, positioning, self to ensure safety to candidate and resident. Count to three.
  12. On signal, assist to stand. (Holding transfer belt cuffed upwards)
  13. Assist client to turn to stand in front of wheelchair with back of clients leg against wheelchair.
  14. Lowers client into wheelchair.
  15. Positions client with hips touching back a wheelchair and transfer belt is removed.
  16. Position feet on foot rests.
  17. Signal light, wash hands.

  3. Residents arm is position with palm up an upper arm exposed.
  4. Feel brachial artery on inner aspect of arm
  5. Places blood pressure
  6. Put on ear pieces of stethoscope/bell is on brachial artery.
  7. Inflate coffee between 160 to 180.
  8. Deflates cuff slowly
  9. Remove scuff.
  10. Signal device, wash hands.
  11. Records.

  4. Applies soap to hands.
  5. Lathers all surface of wrists , hands, and fingers producing friction, for at least 20 seconds, keep hands lower than elbows, and the fingertips down
  6. Cleans fingernails by Robbie fingertips against palms of the opposite hand.
  7. Rinse all surfaces of wrist, hands and fingers, keep hands lower than elbows, and the fingertips down.
  8. Uses clean, dry paper towel/towels to dry all surface of fingers, hands and wrists, starting at fingertips, then disposes of paper towel
  9. Uses clean, dry paper towels, to turn off faucet and dispose of paper towel.

  8. Ask resident what he or she would like to eat first.
  9. Use spoon, offer a bite. Tell resident the constant of each spoon.

  11. Ask resident if they are ready for next fight or food beverage
  12. When done, clean resident mouth and hands.
  13. Remove food tray.

Provides Catheter Care for Female

  2. Provide privacy, grab supplies, bring rail up.
  3. Before washing, check water and have resident verify comfort
  4. Don clean gloves
  5. Please Linen protector on the perineal area.
  6. Expose the area I’ll be working with.
  7. Apply soap to wet washcloth.
  8. While holding catheter meatus, without tugging, clean at least 4 inches of the catheter from the meatus, moving down using clean area of the washcloth before each stroke.
  9. While holding catheter at meatus, without talking, using a clean wash cloth, rinses at meatus , 4 inches of catheter from the meatus, moving down using clean area of washcloth for each stroke.
  10. While holding catheter at meatus without tugging, dries at least 4 inches of catheter away from the meatus using a dry washcloth.
  11. Empties, rinses and dries, places in designated dirty area.
  12. Disposes of used linen into soiled linen container and linen protector.
  13. Avoid contact with use linen.
