The purpose of SOP 101 After action reviews and post incident analysis is to establish a process that allows for the
Systematic and/or routine review of emergency incidents. To reinforce effective operations, correct mistakes, and to identify methods to improve incident operations
Types of post incident analysis
Informal
Formal
What is a informal post incident analysis
“Tailboard critique,” review of the incident prior to units leaving the scene
What is a formal post incident analysis
Written in-depth case study of an incident including all related information and details
Informal post incident analysis
An assessment of the incident should be conducted
As soon as possible prior to units being released from the scene, when operationally feasible. Such review is encouraged after all responses
Informal post incident analysis
Maintains a focus on answering the following questions
- What was planned?
- What actually happened?
- Why did it happen?
- What should or should not be done next time?
Informal post incident analysis
The goal is to determine
What when right and/or wrong in order to recognize effective behavior and/or opportunities for improvement
Informal post incident analysis
The focus should be placed on
The “what” of the incident and not the “who”
Formal post incident analysis
An evaluation of a
Significant fire or incident identified by the AOC or higher
PIA
Formal post incident analysis
Events where a PIA may be conducted include
Major hazardous materials incident
Incident where there is a firefighter death/injury requiring ambulance transport
2nd alarm
Incident requiring an emergency evacuation or MAYDAY event
Incidents that involve complicated operations i.e. alternate water supply, unified command, etc
PIA will be conducted by
The assistant chief of training or their designee
Other personnel that can be included in conducting a PIA include the following:
Battalion chief of training
Captain of training
Battalion chief of safety and wellness or their designee
Captain from operations
Additional personnel from operations as requested by the assistant chief of training
Components of the PIA
Incident overview Actions taken Identified strengths Areas of improvement Lessons learned Recommendations
A completed PIA report shall be
Forwarded to the division chief or operations for final approval
Copies of the completed PIA shall also be
Forwarded to the battalion chief of safety and wellness and the assistant chief of training
Completed PIA’s shall be placed
On Firenet for all employees to review

Performance-based data collection process used to track pre-determined metrics on the emergency scene

Identifying training needs, benchmarking, and/or performance trends
