Ischemic Stroke
Caused by Thromotic or embolic event; artery blocked, blood can’t perfuse, becomes ischemic
Hemorrhagic stroke
Caused by aneurysm, HTN, and arteriovenous malformation
Major warning sign of stroke
Transient Ischemic Attack
therapy for thrombotic stroke
Pt prereqs:
- Not having an active bleed
- No stroke or surgery in last 3 months
- No Significant trauma
- 2 sites of IV access
Thrombolytics -Anistrplase -Retiplase Streptase -t-Pa
Anticoagulants
- Heparin/warfarin
- Xarelto
- Eliquis
- Lovenox
Antiplatelet:
-Plavix
Antiepileptics (Lorazepam)
Ca channel blockers
Stool softeners (no bearing down)
Analgesics for pain
Antianxiety drugs
Nursing interventions for stroke
Monitor ICP
Monitor albumin and glucose (fluid shift)
Monitor safety regarding:
- Unilateral neglect
- Poor vision
- Low mobility
- Swallowing issues
- Elimination issues
Medical stroke management
Thrombolytics
Surgery:
- Carotid angioplasty w/stent (scrapes plaque out)
- Endarterectomy
- Clipping of aneurysms
- craniotomy: remove clots, relieve pressure
- Extracranial-intracranial bypass
