Airway in the ICU: Non-invasive & Invasive - 6

Non-invasive ventilation
1. Nasal cannula
2. Oxygen mask
3. NHF (Nasal High Flow)
4. CPAP
Invasive:
1. Endotracheal tube (tube through mouth into airway)
2. Tracheostomy (incision in neck to insert tube through)

Inotropes & Vasopressors - 3
A
- Inotrope – increases contractile force
- Vasopressor – reduces vasodilation
- Both methods aim to raise blood pressure -> increases perfusion

Electrolytes – 3
A
- Blood gases reviewed hourly in acute illness; frequency reduces as patient improves
- Insight into renal function
- Nurse led e.g. PRN potassium, magnesium & phosphate

Renal replacement therapy Indicated for – 5
A
- Acidosis
- Electrolyte imbalance
- Intoxication (e.g. overdose)
- Overload (fluid overload)
- Uraemia (absence of urine)

Anticoagulation – 5
A
- Clotting can occur during filtration
- Use either sodium citrate – calcium binds citrate to form a complex that inhibits the clotting cascade
- Calcium then removed via the filter
- Heparin IV infusion into the patient
- Not suitable if active bleeds, ischaemic stroke or low platelets (or other contraindications)

Fluid balance - 3
A
- Fluid balance is importance = (input vs output)
- Some medicines e.g. co-trimoxazole and labetalol provide high / excess fluid volumes for patient
- Managed via RRT / diuretics

Sepsis actions – 6

- Ensure senior clinician presence
- O2 if required
- Obtain IV access, take bloods
- Give IV Abx
- Give IV fluids
- Monitor
