Define systems review
a brief screening that allows the therapist to scan the patient’s body systems and determine areas of intact function and dysfunction in each of the following systems
- Cardiopulmonary
- integumentary
- musculoskeletal
- neuromuscular/neurologic
- gait
- motor learning
What are the components of the cardiopulmonary part of a systems review?
HR BP RR O2 edema
What are the components of the integumentary part of a systems review?
integrity, scar formation, color
What are the components of the musculoskeletal part of a systems review?
gross ROM
gross isometric strength
What are the components of the neuromuscular/neurologic part of a systems review?
motor control
balance screen
What are the components of the gait part of a systems review?
gait screen
Define edema
an excessive accumulation of interstitial fluid that may be localized or generalized
Define characteristics of generalized edema
shortness of breath, ankle swelling (often B), increased RR
What may cause general edema
CHF
Hepatic cirrhosis
pulmonary edema
Describe +1 pitting edema
barely detectable impression when finger is pressed into skin
Describe +2 pitting edema
slight indentation; 15 seconds to rebound
Describe +3 pitting edema
deeper indentation; 30 seconds to rebound
Describe +4 pitting edema
> 30 seconds to rebound
Define cyanosis
blue-gray discoloration of the nail beds and lips which indicates inadequate oxygenation
What may cause cyanosis?
decreased CO, exposure to cold, arterial or venous obstruction, Raynaud’s
Define Hemosiderin staining
a rusty discoloration of the lower extremities
What may cause Hemosiderin staining?
chronic venous disease
What is ecchymosis?
bruising; blue-purple when new, green-yellow when old
What is erythema?
increased blood flow causing redness
What is jaundice?
a yellow hue, usually noticed in the sclera, due to impaired liver function
What is pallor
paleness; decreased color of the skin usually caused by low hemoglobin or impaired circulation
What is clubbing?
bulbous swelling of the fingertips caused by longstanding hypoxia; associated with cyanosis
What are Beau’s lines?
Deep indented transverse lines across the nail
What causes Beau’s lines
trauma, Raynaud’s disease, psoriasis, infection
