insulin icodec dosing: T2DM new to insulin
70 units weekly
insulin icodec dosing: T2DM already on insulin
LD: 1.5 x basal x 7 days
MD: basal x7
Insulin icodec dosing : T1DM
A1C >/= 8: LD of 2xbasalx7; MD: basal x7
A1C< 8: LD of 1.5x basalx 7 , MD: basalx7
Insulin Basal doing T2DM
10 units at HS + 1 unit/day as needed to reach FPG 4-7
if degludec increase 2 units every 3-4 days
T2DM: one bolus dosing
- once daily with largest meal (10% basal or 4 units)
- use 2hPPG to measure efficacy
- stop SUs and M
gliclazide IR dosing
80-320mg once daily to BID (30 mins before meals)
- not at HS
gliclazide MR dosing
Once daily in the am
- 30-120mg
Metformin dosing: IR vs ER
IR: 250mg once daily or BID ; increase every 7 days (MDD; 2000mg)
ER: once daily dosing 500-2000mg taken in the evening
Acarbose dosing
dosed with meals —- taken with first bit of food
- 25-100mg divided TID
Repaglinide dosing
0.5-4mg with each meal (MDD 16mg)
- take 15 mins before eat
- skip if don’t eat
Pioglitazone dosing
Once daily
15-45mg
- must be taken at same time each day
Lingaliptin dosing
5mg once daily (Trajenta)
- don’t need to dose adjust in RF
Sitagliptin dosing
25-100mg once daily (Januvia)
- taken at any time during the day
What condition should you avoid saxagliptin in
HF
Dulaglitide dosing
SC weekly - at same day + time (Trulicity)
- 0.75-1.5mg SC
Liraglutide dosing
SC daily at same time (Victoza)
- 0.6-1.8mg SC
semaglutide SC dosing
0.25mg SC weekly x4 weeks then increase to 0.5mg SC weekly
Rybelsus dosing
PO, 30 mins before first meal of the day with water
7-14mg once daily
Canagliflozin dosing
100-300mg once daily before 1st meal
