Action of sulfonylureas

A
  • Increase insulin secretion from B cells (phase 1 spike)
  • Act by INHIBITING K(ATP) channels
  • Orally absorbed
  • Excreted via kidney
  • Adverse effects: hypoglycaemia, weight gain

What do biguanides do?

A
  • Increase insulin mediated peripheral glucose uptake
  • decrease hepatic glucose production
  • cause small decrease in LDL level and TAGs
  • acting by ACTIVATING AMP kinase
  • adverse effects: GI disturbances, no weight gain. Contraindicated in pts with impaired renal function
