A

Created by Lawrence Weed in the 1960s as part of his recommendation for a problem-oriented medical record. This is a commonly accepted and used part of the medical record today. At a high level, the problem list states the most important health problems facing the patient. Beyond the patient visit, it can be used to identify disease-specific populations.

SW and PSF: Published December; 12-month snapshot, gives overall market segment; also Mid Year Top 20 Report published in June but no Best in KLAS awards. ME Top published in June with the Mid-Year published in December.
