1
Q

Why theory driven implementation science?

A

Implementation is tough and we want to increase that our efforts will be successful

Q

Goal (Benefits) of models, theories and frameworks?

A

Shape efforts to translate EBI into practice improvements
Also:
* Critical and comprehensive thinking
* Supports the replication of successful implementations
* Helps navigate complex implementation efforts
* Builds knowledge about implementing intevention
* Identify and understand barriers and facilitators
* Informs hypothesis and implementation of strategies and measures
* Understanding of processes
* Guide rigorous selection of measures and outcomes

Q

MTF

A

Inform the identification of implementation strategies and measures.

What do I want the TMF to do?

There are at least 61, many to choose from.

Q

Model

A

Simplification of a phenomenon or aspect of it

They are theories with more narrowly define scope of explanation
Less generizable to other phenomenon

Q

Theory

A

Analytical principles or statements designed to structure our observation, understanding and explanation of the world

Provide explanation of how and why relationships lead to specific events

Q

Frameworks

A

Structure, overview or outline how concepts are related and presumed to influence a phenomenon

Does not provide explanations, describe fenomena and fits them into sets of categories

Q

Classifications VA perspective

A
  1. Prescriptive/ Process/ Planned Action models: for user how implementation should be planned, org and scheduled
  2. Explanatory/Impact/Descriptive models: how change occurs, including related influences
  3. Mixed models: Both process process/planned, descriptive/explanaotory

Q

Classification MTF

A
  1. Process models: describe/guide process of translateign research into pracctice, steps, stages, phases (Knowledge to action)
  2. Determinants framewiorks: general determinants that are thought to infleucne implementation outcomes (barries and facilitators: CFIR)
  3. Classic theories: from other fields beyond implementation science (diffusion of innovations curve)
  4. Implementation theories: enhance understanding and explanation of certain aspects of implementation (REP by CDC for HIV pre-impl-dissemin, or Organizational readiness for change)
  5. Evaluation framewokrs: structure for evaluation implementation endeavors (RE AIM, reach, effectiveness, adoption, implementation and maintenance)

Q

CFIR

A

Consolidated Framework for implementation Research

  • What works, where and why accross contexts
  • Implementation theories but not healthcare
  • 5 domains (Impl. characteristic, outer settings, inner settings, individual, process)
  • It is flexible, constructs can be chosen,
  • standarizes constructs, organize findings accross implementations

Q

Choosing a MTF

A
  1. What do you need it to do?
  2. Do origins matter
  3. Which level?
  4. Previous findings, experience
  5. Potential for adding to the knowledge base
  6. Extent of guidance or flexibility needed
  7. Location on the dissemination to implementation spectrum

Whatever TMF ypu choose it should be carried trhought entire project, study design, activities, methos and measures, evaluation

Many tools but checklists and tools help narrow it down.

Q

Existing or New
MTF

A

Existing:
* Encourages to build on previous findings
* New application increases generalizability
* Enhances understanding of a model and its constructs
* Developed by utilizeing others developed in other disciplines
* Prevents duplication across disciplines
* Using a mdoel ensure appropiatnes for proposed intervention, cultural preferences of the target

New:
* Affirm that the new model truly addresses a gap in the literature

You can also adapt existing one, it is common and improves appropiatness depedning on interention, population, setting, contributes to the field by testing modifications to existing models
